目的研究玻璃体腔注射贝伐单抗治疗糖尿病黄斑水肿(DME)的疗效及其对患眼视功能的影响。方法选取开封市祥符区中医院2013年4月—2014年11月确诊收治的84例DME患者,对照组给予玻璃体腔注射曲安奈德治疗,观察组采用玻璃体腔注射贝伐单抗治疗。密切随访比较两组治疗前、治疗12周后的中央黄斑厚度(CMT)、黄斑总体积(TMV)和视力表LogMAR得分。结果两组治疗均取得一定积极疗效,但组间相较可知观察组治疗12周后CMT(284.6±105.4)μm、TMV(9.8±1.95)mm~3较对照组(374.5±121.5)μm、(10.7±2.03)mm~3明显较低,治疗后视力检测LogMAR(0.34±0.07)较对照组(0.91±0.09)也显著较高,均具有统计学意义(P<0.05);观察组玻璃体腔注射后12周内的不良反应率7.14%较对照组23.91%明显较低,具有统计学意义(P<0.05)。结论玻璃体腔注射贝伐单抗能有效改善糖尿病黄斑水肿患者的黄斑肿胀症状,提高患者的视力,且不良反应较少,短期内治疗效果较为显著,但本次研究缺乏长期随访数据,其远期疗效和安全性需进一步验证。
Objective To investigate the efficacy of intravitreal bevacizumab in the treatment of diabetic macular edema (DME) and its effect on ocular function. Methods Eighty-four DME patients diagnosed in Xiangfu District Chinese Medicine Hospital of Kaifeng City from April 2013 to November 2014 were selected. The control group was treated with intravitreous injection of triamcinolone acetonide. The observation group was treated with intravitreal injection of bevacizumab. The follow-up was compared between the two groups before treatment, after 12 weeks of treatment of central macular thickness (CMT), total macular volume (TMV) and visual acuity LogMAR score. Results After treatment, CMT (284.6 ± 105.4) μm and TMV (9.8 ± 1.95) mm ~ 3 in the observation group were significantly higher than those in the control group (374.5 ± 121.5) μm and ( 10.7 ± 2.03) mm ~ 3 was significantly lower, and LogMAR (0.34 ± 0.07) after treatment was significantly higher than that of the control group (0.91 ± 0.09), both of which were statistically significant (P0.05) The rate of adverse reactions in the last 12 weeks was 7.14% lower than that in the control group (23.91%), with statistical significance (P <0.05). Conclusion The intravitreal injection of bevacizumab can effectively improve the macular swollen symptoms and improve the visual acuity of patients with diabetic macular edema, with less adverse reactions and short-term therapeutic effects. However, the long-term follow-up data are lacking in this study Efficacy and safety need further verification.
