患者,男性,28岁,于1979年10月27日入院。2天前开始头痛、畏寒、发热、周身不适。入院前一天头痛加剧,体温40℃,入院当日呕吐3～4次为黄色液体胃内容物,精神萎,烦躁,不思饮食。入院体检:体温39.9℃,脉搏80次/分,呼吸22次/分,血压100/70毫米汞柱,神清,精神萎糜,急性痛苦貌,除口腔颊粘膜有少数出血点外,余未见瘀点、瘀斑及玫瑰疹。两颌下均可扪及如蚕豆大小淋巴结,滑动无粘连。舌质红,苔黄腻,咽粘膜充血,颈软,心率80次/分,律齐,肺部呼吸音清晰。腹软、肝肋下未及,剑下3.5～4 cm 质Ⅰ—Ⅱ°,脾肋下1 cm 质Ⅱ°。两侧腰大肌部叩击痛(+),病理反射阴性。
