目的评价以急性腹痛为首发症状的睾丸扭转,以引起临床医生的高度重视。方法回顾性分析我院收治的31例睾丸扭转,其中1例睾丸扭转以急性腹痛为首发症状。结果 26岁男性患者因夜间突发转移性右下腹痛2h入院,经急诊室初步诊断为急性阑尾炎收入普通外科进行抗感染治疗,1d后出现右侧睾丸疼痛,行彩色多普勒超声发现右侧睾丸血流信号消失,转入泌尿外科行右侧睾丸切除术,左侧睾丸固定术。结论临床医生要警惕以腹痛为首发症状的睾丸扭转,彩色多普勒超声能准确判断睾丸扭转等急症,是睾丸扭转的首选检查,为了挽救睾丸,一旦确诊或者可疑睾丸扭转,有必要立即手术探查,手术探查是诊治睾丸扭转最可靠的方法。
Objective To evaluate the testicular torsion of acute abdominal pain as the first symptom to arouse the great attention of clinicians. Methods Retrospective analysis of 31 cases of testicular torsion in our hospital, including 1 case of testicular torsion in acute abdominal pain as the first symptom. Results The 26-year-old male patient was hospitalized for 2 hours due to sudden right lower quadrant pain at night. The patient was admitted to the emergency room for general anti-infective therapy. The right testicular pain appeared on the 1st day. The right side of the right side of the color Doppler ultrasound was found Testicular blood flow signal disappeared, transferred to the urology right orchidectomy, left testicular fixation. Conclusion Clinicians should be wary of testicular torsion with abdominal pain as the first symptom. Color Doppler ultrasound can accurately determine the emergency of testicular torsion. It is the first choice of testicular torsion. In order to save the testicle, once the diagnosis or suspicious testicular torsion is necessary, immediate surgical exploration is needed Surgical exploration is the most reliable method of diagnosis and treatment of testicular torsion.
