目的:观察维持性血液透析(MHD)患者血红蛋白变异度和血清白蛋白的关系及分析其相关因素,为MHD患者肾性贫血的诊治提供新的思路。方法:选择在佛山市禅城区中心医院血液透析中心符合纳入标准的MHD患者110例,收集基本临床资料、各项相关生化指标,计算血红蛋白变异相关指标。按血清白蛋白是否达标分组,分为1组(白蛋白正常组≥35 g/L)和2组(低白蛋白组<35 g/L)进行统计分析。结果:⑴与1组比较,2组患者体重指数、血肌酐、转铁蛋白饱和度均明显降低[(20.78±3.45)kg/mn 2 vs (22.87±3.91)kg/mn 2;(913.06±175.05)μmol/L vs (1 039.20±285.43)μmol/L;(24.83±14.36)% vs (30.39±10.38)%,n P0.05)。⑵与1组比较,2组血红蛋白平均值更低、达标次数更少、血红蛋白最小值更小,血红蛋白变异系数更大,差异均有统计学意义(n P0.05)。⑶Pearson相关分析显示,血清白蛋白与血红蛋白平均值、达标次数、血红蛋白最小值呈正相关(n r=0.401,0.378,0.200,n P<0.05);与血红蛋白变异系数呈负相关(n r=-0.237,n P=0.013)。n 结论:维持性血液透析合并低白蛋白血症患者血红蛋白水平更低,变异度更大,改善营养状态或可提高血红蛋白水平并减小其变异度。“,”Objective:To observe the relationship between hemoglobin variability and serum albumin in maintenance hemodialysis (MHD) patients, and analyze the related factors.Methods:110 MHD patients who met the inclusion criteria in the hemodialysis center of Chancheng District Central Hospital of Foshan City were selected. The basic clinical data, biochemical indexes and hemoglobin variation related indexes were collected. The patients were divided into group 1 (normal albumin group ≥35 g/L) and groups 2 (low albumin group <35 g/L) according to serum albumin level.Results:⑴ Compared with group 1 (normal albumin group), the body mass index (BMI), serum creatinine and transferrin saturation in group 2 (low albumin group) were significantly decreased [(20.78±3.45)kg/mn 2 vs (22.87±3.91)kg/mn 2; (913.06±175.05)μmol/L vs (1 039.20±285.43)μmol/L; (24.83±14.36)% vs (30.39±10.38)%,n P0.05). ⑵ Compared with group 1, the group 2 had lower mean hemoglobin value, fewer times of reaching the standard, lower hemoglobin minimum value and larger hemoglobin coefficient of variation (n P0.05). ⑶ Pearson correlation analysis showed that serum albumin was positively correlated with mean hemoglobin, times of reaching the standard and minimum hemoglobin (n r=0.401, 0.378, 0.200, n P<0.05), and negatively correlated with coefficient of variation of hemoglobin (n r=-0.237, n P=0.013).n Conclusions:Maintenance hemodialysis patients with hypoalbuminemia have lower level of hemoglobin and greater coefficient of hemoglobin variability.Improving nutritional state maybe increase hemoglobin level and decrease coefficient of hemoglobin variability.
