目的分析两种肠造口术治疗老年梗阻性乙状结肠癌与高位直肠癌的临床疗效。方法 60例术后诊断为乙状结肠癌或高位直肠癌患者,按照不同术式分为结肠造口术组与回肠袢式造口组,各30例。对两组患者Ⅰ期、Ⅱ期手术住院时间、手术时间、术后禁食时间及并发症等进行分析。结果Ⅰ期手术,回肠袢式造口组禁食时间短于结肠造口术组,差异有统计学意义(P<0.05)。Ⅱ期手术,回肠袢式造口组手术时间、禁食时间、住院时间短于结肠造口术组,差异有统计学意义(P<0.05);回肠袢式造口组术后低钾发生率明显低于结肠造口术,差异有统计学意义(P<0.05)。结论对于老年梗阻性乙状结肠癌和高位直肠癌患者采取回肠袢式造口术治疗,优势明显,具有临床应用价值。
Objective To analyze the clinical effects of two kinds of enterostomy in the treatment of senile obstructive sigmoid colon cancer and high rectal cancer. Methods Sixty patients diagnosed as sigmoid colon cancer or high rectal cancer were divided into colostomy group and ileal ostomy group according to different surgical procedures, 30 cases in each group. The hospitalization time, operation time, postoperative fasting time and complications of stage Ⅰ and stage Ⅱ in two groups of patients were analyzed. Results The duration of fasting in stage Ⅰ operation and ileum 袢 type ostomy group was shorter than that in colostomy group, the difference was statistically significant (P <0.05). The operation time, fasting time and length of hospital stay in stage Ⅱ operation and ileum 袢 type ostomy group were shorter than that in colostomy group (P <0.05). The incidence of postoperative low potassium in 袢 type ostomy group Significantly lower than colostomy, the difference was statistically significant (P <0.05). Conclusion The treatment of obstructive sigmoid colon cancer and high rectal cancer in elderly patients with ileal 袢-style ostomy has obvious advantages and has clinical value.
