守门人(gatekeeper)的初始意义是政府、保险公司或付费机构(第三方付费方)为了给居民或参保人支付保险费用时,由家庭医生对居民或参保人的疾病诊治活动及费用进行“把关”。开始的目标就是为了控制费用,是从第三方费用控制的角度来考虑的,后来随着服务理念的发展与深入,守门人由原来的“费用控制”概念延伸到了“健康管理”的内涵,但目前“守门人”概念往往仍偏向第三方付费方的利益。
