目的探讨钼靶及超声乳腺影像报告和数据系统(BI-RADS)对乳腺原位癌的早期诊断价值。方法选择2015年1月-2017年1月在威海市立医院接受诊疗的122例乳腺原位癌患者为观察组。另纳入病理学检查明确为乳腺良性包块的145例患者作为对照组。两组患者均行乳腺钼靶及超声检查,分别评估超声BI-RADS分类及钼靶BI-RADS分类对乳腺原位癌的诊断价值。结果观察组患者钼靶BI-RADS分类及超声BI-RADS分类比较,差异无统计学意义(t=1.279,P=0.202);对照组患者钼靶BI-RADS分类及超声BI-RADS分类比较,差异无统计学意义(t=0.314,P=0.754)。观察组患者钼靶BI-RADS分类显著高于对照组(t=12.700,P<0.01);观察组超声BI-RADS分类显著高于对照组(t=15.070,P<0.01)。ROC曲线分析显示,以4为截点值,超声BI-RADS诊断原位癌的敏感性为88.97%,特异性为81.97%,曲线下面积为0.89,略优于钼靶BI-RADS。相关性分析显示,观察组患者观察组患者钼靶BI-RADS及超声BI-RADS评分间呈现显著正相关关系(r=0.479,P<0.01)。结论在乳腺原位癌的早期诊断中超声BI-RADS及钼靶BI-RADS均具有较高价值,且两者具有良好的相关性。
Objective To investigate the value of mammography and ultrasound imaging and data system (BI-RADS) in the early diagnosis of carcinoma of the breast in situ. Methods A total of 122 patients with carcinoma of the breast in situ in Weihai Municipal Hospital from January 2015 to January 2017 were selected as the observation group. Another included in the pathological examination of breast benign masses of 145 patients as a control group. Two groups of patients underwent mammography and ultrasound examination, respectively, to assess the classification of ultrasound BI-RADS and mammography BI-RADS classification of breast cancer in situ diagnosis. Results There was no significant difference in the classification of BI-RADS and BI-RADS between two groups (t = 1.279, P = 0.202) in the observation group. The BI-RADS classification and ultrasound BI-RADS classification in the control group were compared, The difference was not statistically significant (t = 0.314, P = 0.754). The BI-RADS classification in the observation group was significantly higher than that in the control group (t = 12.700, P <0.01). The classification of BI-RADS in the observation group was significantly higher than that in the control group (t = 15.070, P <0.01). ROC curve analysis showed that the sensitivity and specificity of BI-RADS in diagnosing carcinoma in situ were 88.97%, 81.97% and 0.89, respectively, which were slightly better than those of BI-RADS. Correlation analysis showed that there was a significant positive correlation (r = 0.479, P <0.01) between the BI-RADS and BI-RADS scores in the observation group and the observation group. Conclusion Ultrasound BI-RADS and molybdenum target BI-RADS are of high value in the early diagnosis of carcinoma of the breast, and the two have a good correlation.
