目的:应用Meta分析探讨基质金属蛋白酶-1(MMP-1)基因启动子区1607位点单核苷酸多态性(SNP)与头颈癌(HNC)的易感相关性。方法:在中英文数据库中检索MMP-1与HNC相关性研究的文献,根据纳入与排除标准筛选文献,获取截止2014年1月以前公开发表的亚洲人群病例对照研究资料,应用Stata 10.0软件对研究结果进行异质性检验和效应值合并,并进行敏感性分析和偏倚评估。结果:9篇文献纳入研究,累计病例2 049例,对照2 158例;基于随机效应模型的Meta分析显示,MMP-1(-1607)1G/2G多态性与HNC相关性中基因型1G2G+2G2Gvs.1G1G(OR=1.45,95%CI为1.25~1.68,P<0.01),2G2Gvs.1G1G+1G2G(OR=1.77,95%CI为1.37~2.30,P<0.01),2G等位基因vs.1G(OR=1.52,95%CI为1.26~1.85,P<0.01),2G2Gvs.1G1G(OR=2.06,95%CI为1.41~3.01,P<0.01)。结论:MMP-1(-1607)1G/2G多态性与HNC易感性存在关联,2G2G基因型携带者为HNC的易感人群。
OBJECTIVE: To investigate the susceptibility of single nucleotide polymorphisms (SNPs) at the 1607 locus of the matrix metalloproteinase-1 (MMP-1) gene promoter to head and neck cancer (HNC) using meta-analysis. METHODS: The literature on the correlation between MMP-1 and HNC was searched in Chinese and English databases. The literature was screened according to inclusion and exclusion criteria. Case-control studies for Asian populations published before January 2014 were obtained. Stata 10.0 software was used for the study. The results were tested for heterogeneity and effect values combined, and sensitivity analysis and bias assessment were performed. Results: 9 articles were included in the study, cumulative cases were 2 049 cases and 2 158 controls; Meta analysis based on random effects model showed that MMP-1(-1607) 1G/2G polymorphism was associated with HNC genotype 1G2G+ 2G2Gvs.1G1G (OR=1.45, 95% CI, 1.25~1.68, P<0.01), 2G2Gvs.1G1G+1G2G (OR=1.77, 95% CI, 1.37 to 2.30, P<0.01), 2G allele vs. 1G (OR=1.52, 95%CI 1.26~1.85, P<0.01), 2G2Gvs.1G1G (OR=2.06, 95%CI 1.41~3.01, P<0.01). CONCLUSION: MMP-1(-1607) 1G/2G polymorphism is associated with susceptibility to HNC, and 2G2G genotype carriers are HNC susceptible individuals.
