目的分析肺硬化性血管瘤(Pulmonary Sclerosing Hemangioma PSH)的影像及临床病理学形态特征,以提高对该病影像征象的认识,减少误诊。方法对本院经过手术病理证实的5例肺硬化性血管瘤的影像图像及病理资料进行分析。结果 5例均为单发病灶,靠近肺的边缘部。病灶呈圆形、类圆形肿块或结节影,边缘清楚,无深分叶征、血管切迹征,平扫密度较均匀,CT增强扫描肿瘤明显均匀强化,灌注曲线为持续平台型。瘤肺界面清晰,病灶周围肺组织清晰,1例见"空气新月征。结论 PSH在CT平扫中具有一般良性肿瘤的特点,靶扫描及增强扫描尤其是动态增强CT扫描对其诊断有较高价值,结合病理特征有助于硬化性血管瘤的诊断。
Objective To analyze the imaging and clinicopathological characteristics of pulmonary sclerosing hemangioma (PSH) in order to improve the understanding of the imaging signs and reduce the misdiagnosis. Methods The imaging data and pathological data of 5 cases of pulmonary sclerosing hemangioma confirmed by surgery and pathology in our hospital were analyzed. Results 5 cases were single lesions, close to the edge of the lungs. The lesions were round, with round lumps or nodules, clear margins, no deep lobulation, signs of vascular incision and even density. The tumors were significantly and uniformly enhanced by CT-enhanced scanning. The perfusion curve was a continuous plateau. The lung-lung interface was clear, the lung tissue around the lesion was clear, and in 1 case, the air crescent sign was found.Conclusion PSH has the characteristics of normal benign tumors in CT plain scan.However, the target scan and enhanced scan, especially the dynamic enhanced CT scan, Higher value, combined with pathological features contribute to the diagnosis of sclerosing hemangiomas.
