目的探讨血清癌胚抗原(CEA)与血清铁蛋白(SF)联合检测对男性肺腺癌的诊断价值。方法采用电化学发光免疫分析法检测267例男性肺腺癌患者及58例健康男性的CEA、SF水平。应用Logistic回归分析和受试者工作特性曲线(ROC曲线)对检测结果进行分析评价。结果男性肺腺癌组CEA、SF水平显著高于对照组,差异有统计学意义(P<0.05)。以健康男性为对照,男性肺腺癌CEA:AUC=0.912,cut-off值为3.39μg/L,诊断敏感度为76.8%,特异度为94.8%;SF:AUC=0.636,cut-off值为397μg/L,诊断敏感度为45.3%,特异度为87.9%。CEA联合SF的预测概率Y敏感度为81.3%,特异度为93.1%。结论 CEA与SF联合诊断可提高男性肺腺癌诊断的敏感度,在临床上有一定的实用价值。
Objective To investigate the diagnostic value of combined detection of serum carcinoembryonic antigen (CEA) and serum ferritin (SF) on lung adenocarcinoma in men. Methods Electrochemiluminescence immunoassay was used to detect CEA and SF levels in 267 male patients with lung adenocarcinoma and 58 healthy men. Logistic regression analysis and receiver operating characteristic curve (ROC curve) were used to analyze and evaluate the test results. Results The levels of CEA and SF in male lung adenocarcinoma group were significantly higher than those in control group (P <0.05). In healthy men, CEA of the lung adenocarcinoma was 0.912, the cut-off value was 3.39μg / L, the diagnostic sensitivity was 76.8% and the specificity was 94.8%. The SF-AUC was 0.636 and the cut-off value was 397μg / L, the diagnostic sensitivity was 45.3% and the specificity was 87.9%. The predictive probability Y of CEA combined with SF was 81.3% and the specificity was 93.1%. Conclusion The combination of CEA and SF can improve the diagnostic sensitivity of pulmonary adenocarcinoma in men and has certain practical value in clinic.
