目的探讨热休克蛋白27(HSP27)、三叶因子Ⅱ(TFF2)在胃癌和癌前疾病中的表达及临床意义。方法应用免疫组化法检测经病理证实的正常胃黏膜组织20例、胃溃疡22例、慢性萎缩性胃炎24例和胃癌54例中HSP27和TFF2的表达情况,其表达水平以HSP27和TFF2的平均光定密度测定值表示,并对相关资料进行统计学分析。结果HSP27在正常胃黏膜中的表达光密度值(0.278±0.011)较胃溃疡(0.303±0.013)和慢性萎缩性胃炎(0.305±0.010)低,慢性萎缩性胃炎与正常胃黏膜组织中HSP27表达水平比较差异有统计学意义(P<0.05);HSP27在胃癌中的表达水平(0.318±0.022)较前三者高,差异均有统计学意义(P均<0.05);HSP27的表达与胃癌分化程度、淋巴转移有关(P均<0.05)。TFF2在胃溃疡(0.368±0.014)和慢性萎缩性胃炎(0.371±0.019)中的表达水平较正常胃黏膜(0.346±0.032)高,差异有统计学意义(P均<0.05)。胃癌中TFF2的表达水平(0.325±0.020)低于前3者,差异有统计学意义(P均<0.05);TFF2的表达与胃癌分化程度、浸润深度有关(P均<0.05)。结论HSP27和TFF2与肿瘤的发生密切相关,检测该指标可为胃癌诊断、判断预后和指导治疗提供依据。
Objective To investigate the expression and clinical significance of heat shock protein 27 (HSP27) and trefoil factor Ⅱ (TFF2) in gastric cancer and precancerous diseases. Methods The expressions of HSP27 and TFF2 were detected by immunohistochemistry in 20 cases of normal gastric mucosa, 22 cases of gastric ulcer, 24 cases of chronic atrophic gastritis and 54 cases of gastric carcinoma. The expression levels of HSP27 and TFF2 were determined by the average of HSP27 and TFF2 Light density determination of values that, and statistical analysis of relevant information. Results The optical density of HSP27 expression in normal gastric mucosa was lower than that in gastric ulcer (0.303 ± 0.013) and chronic atrophic gastritis (0.305 ± 0.010) (0.278 ± 0.011). HSP27 expression in chronic atrophic gastritis and normal gastric mucosa (P <0.05). The expression level of HSP27 in gastric cancer (0.318 ± 0.022) was higher than that in the former three groups (all P <0.05). The expression of HSP27 was correlated with the differentiation degree of gastric cancer , Lymph node metastasis (all P <0.05). The expression of TFF2 in gastric ulcer (0.368 ± 0.014) and chronic atrophic gastritis (0.371 ± 0.019) was significantly higher than that in normal gastric mucosa (0.346 ± 0.032), the difference was statistically significant (all P <0.05). The expression of TFF2 in gastric cancer was lower than that of the former three (P <0.05). The expression of TFF2 was correlated with the degree of differentiation and depth of invasion (all P <0.05). Conclusion HSP27 and TFF2 are closely related to tumorigenesis. Detection of this index may provide a basis for the diagnosis, prognosis and guidance of gastric cancer.
