目的探讨颅内动脉瘤破裂后,患者血清促肾上腺皮质激素、甲状腺激素水平的变化情况。方法监测2010年3月-6月共51例动脉瘤患者术前、术后2~3、4~5 d促肾上腺皮质激素(ACTH)、甲状腺激素水平的变化情况,并对监测结果进行统计学处理。结果 ACTH、促甲状腺激素、甲状腺素、血清游离甲状腺素在术前、术后2~3、4~5 d差异无统计学意义(P>0.05),三碘甲状腺原氨酸(T3)、血清游离三碘甲状腺原氨酸(FT3)在术前、术后2~3、4~5 d差异有统计学意义(P<0.05);不同部位动脉瘤患者的T3、FT3在术前、术后2~3、4~5 d差异无统计学意义(P>0.05);Fisher、Hunt分级不同级别的动脉瘤患者的T3、FT3在术前、术后2~3、4~5 d差异有统计学意义(P<0.05)。结论动脉瘤破裂患者的血清T3、FT3的变化水平情况,可以作为判断患者病情的轻重与预后的一项重要指标。
Objective To investigate the changes of serum adrenocorticotropic hormone and thyroid hormones in patients with ruptured intracranial aneurysms. Methods The changes of adrenocorticotropic hormone (ACTH) and thyroid hormones in 51 aneurysms from March 2010 to June 2010 were monitored before and 2 to 3 days after operation. T3 and FT3 in patients with aneurysm at different sites were significantly higher than preoperative and postoperative There was no significant difference between 2 ~ 3 and 4 ~ 5 d (P> 0.05). The difference of T3 and FT3 in patients with different grades of aneurysm classified by Fisher and Hunt was statistically significant at preoperative and postoperative 2 ~ 3, 4 ~ 5 days Significance (P <0.05). Conclusion The changes of serum T3 and FT3 levels in patients with aneurysm rupture can be used as an important index to judge the severity and prognosis of patients.
