目的:分析中医科医生临床用药安全管理中存在的问题,并提出针对性的干预措施,旨在提升中医科医生临床用药安全。方法:回顾性分析2014年1~12月期间500张中医科医生开具中药处方,分析处方中存在的问题,并于2015年1~12月期间开展中医科医生临床用药安全管理干预措施,确保处方中开具准确的药物、剂型、制药方式与剂量。比较干预前后问题处方发生率。结果:干预前,500张处方中有105张存在问题,其中选择药材不当28例、制药方法不当32例、药物配比不当25例、用药剂量或用药时间不当20例,问题处方发生率为21.0%;干预期间共计开出处方540张,其中有27张存在问题,其中选择药材不当5例、制药方法不当6例、药物配比不当7例、用药剂量或用药时间不当9例,问题处方发生率为5.0%,干预前后中药处方有效率比较差异显著(P<0.05)。结论:不合理的药方可能影响中药治疗的临床疗效,因此,在临床用药过程中,要加强各环节的安全管理,重视提升中医科医生的专业素质和责任心,保障中医科临床用药安全性,提高患者用药安全。
OBJECTIVE: To analyze the existing problems in the safety management of clinical medicine of Chinese medicine practitioners and to put forward targeted interventions aimed at improving the safety of clinical medicine for Chinese medicine practitioners. Methods: A retrospective analysis of 500 Chinese medicine practitioners prescribing traditional Chinese medicine (TCM) prescriptions during January to December 2014 was conducted to analyze the existing problems in prescriptions. Compare the incidence of prescription problems before and after the intervention. Results: Before the intervention, there were 105 problems in 500 prescriptions. There were significant differences in the prescription efficiency of traditional Chinese medicine before and after intervention (P <0.05). Conclusion: Unreasonable prescriptions may affect the clinical efficacy of traditional Chinese medicine treatment.
