研究目的 探讨淋巴细胞糖皮质激素受体、细胞亚群及血清免疫球蛋白与儿童哮喘的关系。 研究设计 病例对照研究 患者和其他参与者 哮喘患儿34例(观察组),对照组34例,性别组成与年龄分布与观察组相同,均为健康儿童。 处理方法 所有患者及参与者均于晨8时外周或股静脉取血,肝素抗凝待测。 测定和主事结果 用受体放射配体结合分析法测定淋巴细胞糖皮质激素受体(GCR),用微量细胞毒法检测T细胞亚群,用半自动生化分析仪测定血清中的免疫球蛋白。观察组和对照组GCR水平分别为3028±1402、5288±2238结合位点/淋巴细胞(t=4.99,P<0.01),CD_3的百分率分别为43.03±7.07、51.26±5.89(P<0.01)、CD_4的百分率分别为29.91±6.47、36.85±7.57(P<0.01),CD_3的百分率分别为18.56±3.68、23.41±6.82(P<0.01),CD_4/CD_3比值分别为1.63±0.31、1.65±0.41(P>0.05),IgA、IgG、IgM水平统计学处理组间无显著性差异。 结论 哮喘患儿静脉血淋巴细胞GCR水平和T细胞各亚群的百分率显著下降,表明哮喘患儿存在着细胞免疫缺陷。提示应用激活GCR的药物,对小儿哮喘的治疗可能有效。
Objective To investigate the relationship between lymphocyte glucocorticoid receptor, cell subsets, serum immunoglobulin and childhood asthma. Study design Case-control study Patients and other participants 34 children with asthma (observation group), 34 patients in control group, sex composition and age distribution were the same as those in observation group, all of whom were healthy children. Treatment Methods All patients and participants in the morning at 8 am or peripheral venous blood, heparin anticoagulant test. Determination and Main Results Lymphocyte glucocorticoid receptor (GCR) was measured by radioligand binding assay using receptor ligand. T cell subsets were assayed by micro-cytotoxicity and serum immunoglobulins were measured by semi-automatic biochemical analyzer. The GCR levels of observation group and control group were 3028 ± 1402,5288 ± 2238 binding sites / lymphocyte (t = 4.99, P <0.01), the percentage of CD3 were 43.03 ± 7.07,51.26 ± 5.89 (P <0.01) The percentages of CD_4 and CD_4 were 29.91 ± 6.47 and 36.85 ± 7.57 respectively (P <0.01), the percentage of CD_3 were 18.56 ± 3.68 and 23.41 ± 6.82 (P <0.01), the ratios of CD_4 / CD_3 were 1.63 ± 0.31 and 1.65 ± 0.41 P> 0.05), there was no significant difference between the two groups in the statistics of IgA, IgG and IgM. Conclusion The GCR level and the percentage of T lymphocyte subsets in the asthmatic children are significantly decreased, indicating that there is cellular immunodeficiency in asthmatic children. Tip application of activated GCR drugs, the treatment of pediatric asthma may be effective.
