目的探讨散发型 Creutzfeldt-Jakob 病(sCJD)的 MR 特点。方法回顾性分析3例临床诊断为 sCJD 的患者资料,MR 采用 SE T_1WI、快速自旋回波(FSE)T_2WI、扩散加权成像(DWI)扫描,观察其 MR 表现特征。结果 3例 SE T_1WI 和 FSE T_2WI 序列对基底节区和皮质的病变显示不佳,DWI 则可清晰地显示病变,额、顶、枕叶皮质最常受累,表现为高信号,病变可对称,也可不对称,皮层下区脑白质信号未见异常。双侧尾状核、丘脑也可受累,DWI 上呈高信号。晚期脑实质广泛萎缩,以皮质为著。结论 sCJD 采用 DWI 序列结合其特征性的临床表现可作出较为准确的诊断。
Objective To investigate the MR characteristics of sporadic Creutzfeldt-Jakob disease (sCJD). Methods The clinical data of 3 patients with clinically diagnosed sCJD were retrospectively analyzed. MR imaging was performed by using SE T 1WI, fast spin echo (TFE) T 2WI and diffusion weighted imaging (DWI). Results Three cases of SE T_1WI and FSE T_2WI showed poor prognosis in basal ganglia and cortex. DWI showed the lesions clearly. The frontal, parietal and occipital lobe cortex were the most frequently affected, showing high signal and symmetrical lesions. May be asymmetric, subcortical white matter signal no abnormalities. Bilateral caudate nucleus, thalamus can also be involved, DWI was high signal. Extensive brain atrophy in late stage, with the cortex as a. Conclusion sCJD using DWI sequences combined with its characteristic clinical manifestations can make a more accurate diagnosis.
