As early as 1934, Dandy first noticed that trigeminal nerve compression caused by the anatomy and pathological structure of the cerebellopontine angle can cause pain. The authors from the clinical point of view explored 52 cases of primary trigeminal neuralgia when the cerebellar pontine angle surgery. Surgery was performed in the lateral position, and the cerebellar pontine angle exploration was performed on a small scope of the skull after the mastoidectomy. All operations were performed under a surgical microscope. Seen from the operation: Arterial spasm compresses the trigeminal nerve and displaces it. 37 patients accounted for 71%. Two cases of arterial spasm were close to the nerves but not in contact. The most common arteries that compressed nerves were the superior cerebellar arteries.
