目的:探讨舌癌化疗的作用机制。方法:60例患者的术后石蜡标本分为3组,其中舌癌化疗组10例,舌癌组30例,舌乳头状瘤组20例。采用原位末端标记法和免疫组织化学染色,并检测舌癌化疗后的细胞增殖、细胞凋亡及Bcl-2表达。结果:舌癌组细胞增殖指数(PI)显著高于乳头状瘤组(P<0.05),而细胞凋亡指数(AI)低于乳头状瘤组(P<0.05);舌癌化疗组与舌癌组比较,AI显著升高(P<0.05),PI显著降低(P<0.05);舌癌化疗组与舌癌组比较,Bcl-2阳性细胞指数明显降低(P<0.05);舌癌化疗组中PI与AI呈负相关(r=-0.739),Bcl-2阳性细胞指数与AI呈负相关(r=-0.787)。结论:化疗药物抑制肿瘤生长的机制可能与抑制肿瘤细胞增殖和诱导肿瘤细胞凋亡有关,这种作用可能是通过下调Bcl-2表达水平实现的,提示舌癌术前化疗对肿瘤的预后有重要意义。
Objective: To explore the mechanism of tongue cancer chemotherapy. Methods: Sixty patients with postoperative paraffin specimens were divided into three groups, of which 10 were tongue cancer chemotherapy group, 30 tongue cancer group and 20 tongue papilloma group. Tumor cell proliferation, apoptosis and Bcl-2 expression after tongue cancer chemotherapy were detected by in situ end labeling and immunohistochemical staining. Results: The proliferation index (PI) in tongue cancer group was significantly higher than that in papilloma group (P <0.05), but the apoptosis index (AI) was lower than that in papilloma group (P0.05) Compared with the cancer group, AI was significantly increased (P <0.05) and PI was significantly decreased (P <0.05); Bcl-2 positive cells index was significantly decreased in tongue cancer chemotherapy group compared with tongue cancer group (P <0.05) There was a negative correlation between PI and AI (r = -0.739) and a negative correlation between Bcl-2 positive cell index and AI (r = -0.787). Conclusion: The mechanism of chemotherapeutic drugs inhibiting tumor growth may be related to inhibiting tumor cell proliferation and inducing tumor cell apoptosis. This effect may be achieved through down-regulating Bcl-2 expression level, suggesting that preoperative chemotherapy of tongue cancer is important for the prognosis of the tumor significance.
