【目的】探讨彩色多普勒超声诊断在中孕期胎儿单脐动脉(SUA)中的诊断价值。【方法】回顾性分析54例中孕期产前超声诊断为胎儿 SUA 孕妇临床资料,探讨胎儿 SUA 超声影像的形态结构、伴发的胎儿畸形与妊娠结局的关系。【结果】54例中孕期 SUA 胎儿在盆腔膀胱及脐轮切面仅见膀胱一侧显示有脐动脉血流信号,另侧血流信号缺失,其中左侧缺失32例,右侧缺失22例。二维超声检查在羊水中游离段脐带横切时见不到正常脐动脉“品字”形结构,而呈“吕”字形结构,彩色多普勒显示为一粗一细,一蓝一红两条血管相间,走形较平直,螺纹较稀疏,S/D 均大于3.0。54例SUA 中12例合并其他部位的畸形,根据其父母意愿引产终止妊娠。其余42例超声诊断为单纯的 SUA,继续妊娠,每隔4周超声追踪观察,其中6例在妊娠晚期出现宫内发育迟缓(IUCR)、早产、心律失常、羊水过多等围产期并发症,余36例妊娠进程顺利,正常分娩,产后随访无明显异常。【结论】超声多普勒产前超声能准确诊断中孕期胎儿 SUA 及其合并的畸形,追踪复查有利于发现 IUCR、宫内缺氧、心律失常等围产期并发症。“,”Objective]To investigate the diagnostic value of color Doppler ultrasound in the diagnosis of fetal SUA in the middle of pregnancy.[Methods]A retrospective analysis of 54 cases of prenatal ultrasound diagnosis of fetal SUA maternal clinical data was conducted to investigate the morphology and structure of fetal SUA ultrasound imaging,and the relationship between fetal malformations and pregnancy outcome.[Results]Among 54 cases in the second trimester of pregnancy SUA fetu-ses,blood flow signal display only on the bladder side umbilical artery in pelvic and umbilical round section;blood flow signal appear loss on the other side,with the absence of 32 cases of leftward and the absence of 22 cases on the right side.Two di-mensional ultrasound examination showed that in the amniotic fluid,The“品 ”-shaped structure of normal umbilical artery was not found in the free section of the umbilical cord,but a“吕”-shaped structure;Color Doppler showed that two vessels ( a thick blue one and an other thin and red )with straight out of shape,and its thread was sparse,S/D was higher than 3.0. Twelve cases of SUA in 54 cases with other parts of the malformation,pregnancy was terminated according to the wishes of their parents.The other 42 cases were diagnosed as pure SUA,followed by ultrasound in 4 weeks,and 6 patients with intrauter-ine growth retardation (IUCR),premature delivery,arrhythmia,amniotic fluid and so on.6 cases occurred in the late trimes-ter of pregnancy (IUCR),premature birth,arrhythmia,amniotic fluid excess and many other complications,The rest 36 ca-ses underwent pregnancy and normal delivery,postpartum follow-up displayed no obvious abnormalities.[Conclusion]Ultra-sonic Doppler prenatal examination can accurately diagnose the fetal SUA and its combined malformation,follow-up review is conducive to the discovery of IUCR,intrauterine hypoxia,arrhythmia and other perinatal complications.
