目的 探讨胰岛素抵抗与脑梗死发病的关系。方法 检测 3 6例脑梗死并高胰岛素血症患者、3 1例脑梗死并正常胰岛素血症患者和 3 2例正常人空腹血糖 (Glu)、血清胰岛素 (INS)、C -肽 (C -P)、血浆总胆固醇 (TC)、甘油三酯 (TG)、载脂蛋白A1(ApoA1)、载脂蛋白B10 0 (ApoB10 0 )、高密度脂蛋白胆固醇 (HDL -C)水平并计算胰岛素敏感性指数 (ISI)。结果 高胰岛素血症脑梗死组胰岛素敏感性指数明显低于正常胰岛素血症脑梗死组及正常对照组 ,而TG、ApoB10 0 明显高于后两组患者 ,HDL -C明显低于后两组患者 ,高胰岛素血症脑梗死组TG、ApoB10 0 与胰岛素敏感性指数呈负相关关系。结论 胰岛素抵抗并继发性高胰岛素血症与脑梗死的发生及发展密切相关。
Objective To investigate the relationship between insulin resistance and the incidence of cerebral infarction. Methods Thirty - six patients with cerebral infarction and hyperinsulinemia were enrolled in this study. Fasting blood glucose (Glu), serum insulin (INS) and C - peptide (P - P) were measured in 31 patients with cerebral infarction and normal insulinoma and 32 healthy controls. (TC), triglyceride (TG), apolipoprotein A1 (ApoA1), apolipoprotein B10 0 (ApoB10 0) and high density lipoprotein cholesterol (HDL-C) were measured and insulin sensitivity was calculated Index (ISI). Results Insulin sensitivity index in hyperinsulinemic infarction group was significantly lower than that in normal ischemic cerebral infarction group and normal control group, but TG, ApoB10 0 were significantly higher than the latter two groups, HDL -C was significantly lower than the latter two groups There was a negative correlation between TG, ApoB10 0 and insulin sensitivity index in hyperinsulinemia group. Conclusion Insulin resistance and secondary hyperinsulinemia are closely related to the occurrence and development of cerebral infarction.
