目的:评估以精细解剖为基础操作技术在甲状腺癌Ⅵ区淋巴结清扫术中对甲状旁腺功能的保护作用。方法:共行甲状腺癌双侧Ⅵ区淋巴结清扫术223例,术中采用以精细解剖为基础的综合措施者118例,采取传统手术方法者105例。比较两种方法切除淋巴结数目多少,术后并发症发生情况。结果:精细解剖组清扫淋巴结平均(9.5±7.5)枚,传统组平均(11.5±8.5)枚(P>0.05);精细解剖组出现手术相关的甲状旁腺功能减退4例,传统组26例(P<0.05)。结论:在保证治疗有效性的同时,采用以精细解剖为基础的综合措施,可以有效降低甲状腺癌VI区淋巴结清扫术后发生甲状旁腺功能减退的风险。
OBJECTIVE: To evaluate the protective effect of fine-dissection-based technique on parathyroid gland function in Ⅵ lymph node dissection of thyroid cancer. Methods: Totally 223 cases of bilateral Ⅵ lymph node dissection of thyroid cancer were treated. Among them, 118 cases were treated with fine dissection-based comprehensive measures and 105 cases were treated by traditional methods. The number of resected lymph nodes and the incidence of postoperative complications were compared between the two methods. Results: In the fine dissection group, the average number of lymph nodes was 9.5 ± 7.5, which was 11.5 ± 8.5 in the traditional group (P> 0.05). There were 4 cases of operation-related hypoparathyroidism in the fine dissection group, 26 cases in the traditional group P <0.05). CONCLUSIONS: In addition to the effectiveness of the treatment, the combination of fine dissection-based measures can effectively reduce the risk of hypoparathyroidism after lymph node dissection in thyroid cancer.
