目的探讨端粒酶(the human telomerase gene,hTERC)基因在不同宫颈脱落细胞中的表达及与人乳头瘤病毒(humanpa pilloma virus,HPV)感染的关系。方法 2007年10月至2008年10月在中国医科大学附属第四医院采用荧光原位杂交(fluorescent in situ hybridization,FISH)方法 ,检测120例子宫颈脱落细胞(正常宫颈20例、宫颈上皮内瘤变(CIN)128例、CIN227例、CIN317例及浸润性宫颈癌28例)hTERC的表达情况及并进行HPV-DNA分型测定。结果 hTERC基因在正常宫颈、CIN1、CIN2、CIN3及浸润性宫颈癌中的阳性表达率分别为10.00%、21.42%、55.60%、88.24%及92.86%。hTERC基因表达宫颈癌组与CIN各组及正常对照组比较均有统计学意义(P均<0.05),CIN2、3组与正常对照组比较有统计学意义(P<0.01)。宫颈病变与HPV感染的宫颈上皮细胞中CIN1、CIN2、CIN3及宫颈癌的hTERC基因扩增阳性率分别为33.33%、66.67%、87.50%、92.86%,77例HPV阳性患者中,hTERC基因扩增比例为76.62%(59/77);而在23例HPV阴性患者中,hTERC基因扩增比例仅为17.39%(4/23),P<0.05,差异具有统计学意义。结论 hTERC基因的扩增与宫颈病变程度及HPV感染呈正相关,hTERC基因高表达在宫颈癌发生发展中可能起重要作用。
Objective To investigate the expression of the human telomerase gene (hTERC) in different cervical exfoliated cells and its relationship with human papillomavirus (HPV) infection. Methods Totally 120 cases of cervical exfoliated cells (20 cases of normal cervix and 18 cases of cervical intraepithelial neoplasia) were examined by fluorescent in situ hybridization (FISH) at the Fourth Affiliated Hospital of China Medical University from October 2007 to October 2008. (CIN) 128 cases, CIN227 cases, CIN317 cases and invasive cervical cancer 28 cases) hTERC expression and HPV-DNA typing. Results The positive rates of hTERC gene in normal cervix, CIN1, CIN2, CIN3 and invasive cervical cancer were 10.00%, 21.42%, 55.60%, 88.24% and 92.86% respectively. The expression of hTERC gene in cervical cancer group was significantly higher than those in CIN group and normal control group (all P <0.05). There was significant difference between CIN2,3 group and normal control group (P <0.01). The positive rate of hTERC gene amplification in CIN1, CIN2, CIN3 and cervical cancer was 33.33%, 66.67%, 87.50%, 92.86% respectively in cervical lesions and HPV-infected cervical epithelial cells. Among the 77 HPV positive patients, the hTERC gene amplification (76.62%, 59/77). In 23 HPV-negative patients, the hTERC gene amplification rate was only 17.39% (4/23), P <0.05, the difference was statistically significant. Conclusion The amplification of hTERC gene is positively correlated with the degree of cervical lesions and HPV infection. The high expression of hTERC gene may play an important role in the development of cervical cancer.
