目的对无锡市各级医疗机构疟原虫镜检能力建设情况进行分析,为今后疟疾防治工作提供科学依据。方法收集2016年无锡市各级医疗机构镜检员问卷调查数据,建立数据库,并采用Logistic回归对镜检员疟原虫检测能力的影响因素进行分析。结果共调查全市54家医疗机构54名镜检员,男性16人,女性38人,男女性别比0.42∶1;有12名镜检员曾在工作中检出疟原虫,占被调查者的22.22%。多因素分析结果表明:女性镜检员在工作中检测出疟原虫的能力比男性镜检员弱(调整OR=0.11,95%CI:0.02~0.53),中级及以上职称镜检员在工作中检测出疟原虫的能力高于初级职称镜检员(调整OR=5.31,95%CI:1.04~27.19),县乡级医疗机构镜检员在工作中检测出疟原虫的能力比其他机构的镜检员弱(调整OR=0.04,95%CI:0.01~0.98)。结论无锡市各级镜检站均具备一定的疟疾诊治能力,需进一步加强基层医疗机构的镜检能力建设。
Objective To analyze the capacity of microscopic examination of Plasmodium in medical institutions at all levels in Wuxi City to provide scientific basis for malaria control in the future. Methods The questionnaire survey data of medical inspectors at different levels in Wuxi in 2016 were collected, and the database was established. The influencing factors on the detection ability of Plasmodium malaria were analyzed by Logistic regression. Results A total of 54 medical inspectors were surveyed in 54 medical institutions in the city, including 16 males and 38 females, with a sex ratio of 0.42:1. Twelve inspectors had detected malaria parasite at work, accounting for 22.22% of the respondents %. The result of multivariate analysis showed that the ability of female inspectors to detect malaria parasites during work was weaker than that of male ones (adjusted OR = 0.11, 95% CI: 0.02-0.53), and mid-level and above professional inspectors were working The detection ability of malaria parasite was higher than that of the junior professional mirror examiner (adjusted OR = 5.31,95% CI: 1.04-27.19). The inspection of malaria parasite at the county and township medical institutions was more effective than the other institutions Inspectors were weak (adjusted OR = 0.04, 95% CI: 0.01 to 0.98). Conclusion There are certain malaria diagnosis and treatment capacities in all levels of inspection stations in Wuxi City, and it is necessary to further strengthen the microscopic examination ability of primary medical institutions.
