目的研究面肌肌电图(EMG)、脑干听觉诱发电位(BAEP)监护技术在大型听神经瘤手术中对面神经保留的作用。方法经枕下入路行显微手术全切除肿瘤,术中同时监测面EMG、BAEP。结果肿瘤全切率100%,面神经解剖保留率94%,功能保留良好率81%。术末刺激强度≤0.4 mA者提示预后良好。结论大型听神经瘤显微手术中,通过面EMG、BAEP监护可显著提高面神经的功能保留率并能提供客观指标提示预后。
Objective To study the effects of facial muscle electromyography (EMG) and brain stem auditory evoked potential (BAEP) monitoring on facial nerve preservation in large acoustic neuroma surgery. Methods Microsurgical resection of tumors was performed by suboccipital approach. Simultaneous monitoring of facial EMG and BAEP was performed. Results The total tumor resection rate was 100%, the anatomy retention rate of facial nerves was 94%, and the functional retention rate was 81%. The end-of-surgery stimulating intensity of ≤ 0.4 mA suggests a good prognosis. Conclusion In large acoustic neuroma microsurgery, facial EMG and BAEP monitoring can significantly improve the facial nerve function retention rate and provide objective indicators of prognosis.
