目的:分析胃肠道癌患者外周血淋巴细胞p16、cyclin Dl表达的临床意义。方法:取43例健康人为对照组、病例组为73例经病理确诊的胃肠道癌患者。用流式细胞仪检测其外周血淋巴细胞p16和cyclin Dl的表达,用mann-whitney U检验分析病例组及对照组外周血淋巴细胞p16、cyclin Dl表达的差异性及外周血淋巴细胞p16、cyclin Dl在胃肠道癌患者不同分期中表达的差异,并绘制ROC曲线,确定临界值,计算p16、cyclin Dl的特异度和灵敏度。结果:胃肠道癌患者外周血淋巴细胞P16及cyclin D1表达水平均明显高于对照组,两组比较差异有显著性(P<0.001);绘制ROC曲线,复发临界值:P16为3.66;cyclin D1为3.84。灵敏度、特异度、准确率为:P16:80.8%、76.7%、79.3%;Cyclin D1:80.8%、56.8%、71.6%;两指标联合检测并联试验灵敏度达90.4%,串联试验特异度达83.7%。而胃肠道癌患者外周血淋巴细胞P16及cyclin D1表达水平则与临床分期无关(P<0.001)。结论:胃肠道癌外周血淋巴细胞p16、cyclin Dl表达高于正常人群,二者联合检测对胃肠道癌的判断有较高的灵敏度和特异度。
Objective: To analyze the clinical significance of the expression of p16 and cyclin Dl in peripheral blood lymphocytes of patients with gastrointestinal cancer. Methods: Forty-three healthy individuals were selected as the control group, and 73 cases were diagnosed as gastrointestinal cancer patients by pathology. Flow cytometry was used to detect the expression of p16 and cyclin Dl in peripheral blood lymphocytes. Mann-whitney U test was used to analyze the difference of expression of p16 and cyclin Dl in peripheral blood lymphocytes between cases and controls, p16 and cyclin Dl in patients with different stages of gastrointestinal cancer expression differences, and draw ROC curve to determine the critical value, calculate the specificity and sensitivity of p16, cyclinDl. Results: The expression levels of P16 and cyclin D1 in peripheral blood lymphocytes of patients with gastrointestinal cancer were significantly higher than those of the control group (P <0.001), and the ROC curve was drawn. The critical value of recurrence was P66 (3.66) D1 is 3.84. Sensitivity, specificity and accuracy were P16: 80.8%, 76.7%, 79.3%; Cyclin D1: 80.8%, 56.8%, 71.6%, respectively. The sensitivity of parallel assay was 90.4% . The expression of P16 and cyclin D1 in peripheral blood lymphocytes of patients with gastrointestinal cancer was not related to clinical stage (P <0.001). Conclusion: The expression of p16 and cyclin Dl in peripheral blood lymphocytes of patients with gastrointestinal cancer is higher than that in normal people. The combined detection of them has high sensitivity and specificity for judging gastrointestinal cancer.
