本文着重分析我院近7年间收治45例老年前期及老年期自发性蛛网膜下腔出血的不典型临床表现。患者年龄均在51岁以上,临床观察表明:老年患者以头痛为首发症状占45％,15％病例始终无头痛,表现为头昏、眩晕、意识障碍、呕吐、颈肩痛、坐骨神经痛、精神症状、癫痫发作、低热、颅神经损害等。部分患者脑膜刺激征阴性。本文对部分老人蛛网膜下腔出血不典型的原因及CT诊断价值做了进一步分析,以期临床医师减少该病的漏诊、误诊。
This article focuses on the analysis of atypical clinical manifestations of 45 cases of spontaneous subarachnoid hemorrhage in our hospital in the past 7 years. The patients were all over 51 years of age. The clinical observation showed that 45% of the elderly patients had headache as the first symptom and 15% of the patients had no headache. The symptoms were dizziness, dizziness, disturbance of consciousness, vomiting, neck and shoulder pain, sciatica, Symptoms, seizures, fever, cranial nerve damage and more. Some patients with meningeal irritation negative. In this paper, some of the causes of atypical subarachnoid hemorrhage and the value of CT diagnosis were further analyzed in order to reduce the clinicians missed diagnosis, misdiagnosis.
