目的:从基因水平探讨肺癌影像学表现的分子生物学基础。材料与方法:利用免疫组化技术和计算机图像分析系统,测定经手术病理证实之15例肺鳞癌及18例肺腺癌组织中癌基因 c-erbB-2及 bcl-2的表达水平,并分析其表达水平与影像学征象的相关性。结果:肺鳞癌的影像学征象中肿块的大小,边缘清楚率及肺门纵隔淋巴结肿大率与 c-erbB-2的表达水平呈负相关,与 bcl-2的表达水平呈正相关;而三级支气管受累率则与 c-erbB-2呈正相关,与 bcl-2呈负相关。肺腺癌肿块的大小、边缘光滑率与 c-erbB-2的表达水平呈负相关,其肺门和纵隔淋巴结肿大率、三级支气管受累率及胸膜凹陷率与 c-erbB-2呈正相关。结论:癌基因 c-erbB-2及 bcl-2的表达对决定肺鳞、腺癌的影像学表现具有重要的作用。
Objective: To explore the molecular biological basis of lung cancer imaging from the gene level. Materials and Methods: The expression of c-erbB-2 and bcl-2 in 15 cases of lung squamous cell carcinoma and 18 cases of lung adenocarcinoma were confirmed by immunohistochemistry and computer image analysis system. The correlation between their expression levels and imaging signs was analyzed. RESULTS: The size of the tumor, the edge clearness, and the rate of mediastinal lymph node enlargement were negatively correlated with the expression of c-erbB-2, and positively correlated with the expression of bcl-2 in the imaging findings of lung squamous cell carcinoma. The level of bronchial involvement was positively correlated with c-erbB-2 and negatively correlated with bcl-2. The size and smoothness rate of lung adenocarcinoma were negatively correlated with the expression of c-erbB-2, and the rates of hilar and mediastinal lymphadenopathy, grade 3 bronchial involvement, and pleural invagination were positively correlated with c-erbB-2. . Conclusion: The expression of oncogene c-erbB-2 and bcl-2 plays an important role in determining the imaging features of lung squamous cell carcinoma and adenocarcinoma.
