[目的]探讨延边地区朝鲜族和汉族人群胰岛素抵抗(IR)对代谢综合征(MS)及其组分患病危险的影响程度.[方法]在延边地区抽取7 856名朝鲜族和汉族常住居民进行现况调查,并测定MS相关代谢指标.[结果]朝鲜族和汉族HOMA-IR水平及IR检出率间存在明显差异.MS及其组分检出率均随着HOMA-IR水平升高呈升高趋势(P<0.001),而且在IR组的检出率明显高于其他组.朝鲜族和汉族人群IR检出率随着MS异常组分数的增加呈升高趋势(P<0.001).多因素Logistic回归分析结果表明,MS及其各组分的患病危险均随着HOMA-IR水平的升高而升高(P<0.01),朝鲜族人群IR组的OR值依次为79.645(高血糖),2.431(腹型肥胖),1.525(低高密度脂蛋白胆固醇血症),1.427(高三酰甘油血症),汉族人群依次为11.743(高血糖),2.702(低高密度脂蛋白胆固醇血症),1.847(腹型肥胖),1.627(高三酰甘油血症),1.367(高血压),IR对朝鲜族和汉族人群MS患病危险分别为4.649和3.750.[结论]MS及其组分的患病危险均随着HOMA-IR水平升高而呈升高趋势;IR对不同代谢异常的患病危险及其聚集强度的影响程度有所不同,且存在民族差异.
[Objective] To investigate the influence of insulin resistance (IR) on the risk of metabolic syndrome (MS) and its components among Korean and Han nationality population in Yanbian area. [Methods] A total of 7 856 Korean and Han residents And the metabolic index of MS were determined. [Results] The HOMA-IR level and the detection rate of IR were significantly different between Korean and Han nationality.The detection rate of MS and its components increased with the increase of HOMA-IR (P <0.001), and the detection rate in IR group was significantly higher than other groups.The detection rate of IR in Korean and Han population increased with the increase of MS abnormality group (P <0.001). Logistic regression analysis showed that the risk of MS and its components were all increased with the increase of HOMA-IR (P <0.01), and the OR of Korean IR patients was 79.645 (hyperglycemia ), 2.431 (abdominal obesity), 1.525 (low HDL cholesterol), 1.427 (hypertriglyceridemia), 11.743 (hyperglycemia), 2.702 (low HDL cholesterol ), 1.847 (abdomen obesity), 1.627 (hypertriglyceridemia), 1.367 (hypertension), IR for MS prevalence in Korean and Han populations Respectively, 4.649 and 3.750, respectively. [Conclusion] The prevalence of MS and its components increased with the increase of HOMA-IR level. The influence of IR on the risk of different metabolic disorders and their aggregation intensity Different, and there are ethnic differences.
