目的初步探讨汉族上皮性卵巢癌(EOC)患者p53基因5-8外显子突变与“二元论”发病分型间的关系。方法选取2011年8月-2012年12月行手术并病理确诊的四川省汉族原发性EOC患者45例,利用DNA测序检测p53基因5-8外显子突变情况,并依照“二元论”将EOC患者分型,结合患者临床病理,分析各型EOC患者p53基因突变情况。结果Ⅱ型EOC患者p53基因突变频率及效率高于Ⅰ型EOC患者,差异有统计学意义(P<0.01);且发现175位密码子可能是Ⅱ型EOC的突变热点。在临床病理参数方面Ⅱ型EOC恶性程度及输卵管受累率高于Ⅰ型EOC,恶性程度高的临床病理特征患者中p53突变率显著升高。结论 p53基因突变在Ⅱ型EOC发生发展过程中可能发挥重要作用,其中175位密码子可能是Ⅱ型EOC的突变热点。
Objective To investigate the relationship between the mutation of exon 5-8 of p53 gene and the morbidities of “dualism” in Han patients with epithelial ovarian cancer (EOC). Methods Forty-five patients with primary EOC diagnosed by surgery and pathology in Sichuan province from August 2011 to December 2012 were enrolled in this study. Mutations of 5-8 exons of p53 gene were detected by DNA sequencing. According to “Dualism” According to the clinicopathological features of patients with EOC, we analyzed the mutation of p53 gene in patients with EOC. Results The frequency and efficiency of p53 gene mutation in type Ⅱ EOC patients were higher than those in type Ⅰ EOC patients (P <0.01). It was found that codon 175 might be a hot spot mutation in type Ⅱ EOC. In clinical pathological parameters of type Ⅱ EOC malignancy and fallopian tube involvement rate is higher than that of type Ⅰ EOC, a high degree of malignancy in patients with clinically and pathologically characterized p53 mutation rate was significantly higher. Conclusion The p53 gene mutation may play an important role in the development of type Ⅱ EOC. The codon 175 in it may be the hot spot of type Ⅱ EOC mutation.
