目的研究CMNa(甘氨双唑钠)在鼻咽癌放射治疗中的增敏作用及毒副反应。方法自2004年1月至2006年2月首程治疗共58例,随机分为常规组及CMNa组,常规组行单纯根治性放疗,CMNa组行根治性放疗的同时用CMNa增敏。放疗方法先制作面模固定,在模拟机下设面颈联合野放疗,DT36Gy/18次/3.5周,后改设耳前野和鼻前野(或耳后野)放疗总量至DT6872Gy/3436次/6.57周。用药方法CMNa每次按800mg/m2给药,用100ml生理盐水溶解后,于30min内静脉滴注完,于60min内进行常规放射治疗。每周一、三、五用药,至常规放射治疗结束为止。结果CMNa组和常规组原发灶有效率分别为93.3%和82.2%,而颈部淋巴结有效率分别为90.9%和73.3%,远处转移率分别为16.7%和17.7%。结论CMNa具有放射治疗增敏作用,而且CMNa组副作用少,但其长期疗效有待进一步研究。
Objective To investigate the sensitization effect and toxicity of CMNa (sodium glycididazole) in radiotherapy of nasopharyngeal carcinoma. Methods From January 2004 to February 2006, a total of 58 patients were treated with the first course of treatment. The patients were randomly divided into conventional and CMNa groups. The conventional group received simple radical radiotherapy. CMNa group was treated with CMNa while radical radiotherapy. Radiotherapy method to make face mold fixation, face and neck under the simulator were combined field radiotherapy, DT36Gy / 18 times / 3.5 weeks, after the change of anterior placenta and anterior foreskin (or ear field) total radiotherapy to DT6872Gy / 3436 times / 6.57 weeks. Medication method CMNa 800mg / m2 administered at a time, with 100ml of saline dissolved in 30min after intravenous infusion, routine radiotherapy within 60min. Every Monday, Wednesday and Friday medication until the end of routine radiation therapy. Results The effective rates of primary tumor in CMNa group and conventional group were 93.3% and 82.2% respectively, while the effective rates of cervical lymph node were 90.9% and 73.3% respectively, and distant metastasis rates were 16.7% and 17.7% respectively. Conclusion CMNa has the radiosensitizing effect, and CMNa group has fewer side effects, but its long-term efficacy needs further study.
