目的 :探讨左旋精氨酸 (L -Arg)对高原肺水肿 (HAPE)自由基代谢的影响。方法 在海拔 370 0米采用静滴L -Arg治疗HAPE患者 8例 (L -Arg组 ) ,并与吸入低浓度一氧化氮 (NO)混合气治疗 8例HAPE(NO组 )作对照 ,治疗前后分别检测血中超氧化物岐化酶 (SOD)、谷胱苷肽过氧化酶 (GSH -Px)及丙二醛 (MDA)含量。结果 :L -Arg组和NO组治愈后较治疗前SOD、GSH -Px增高 ,MDA降低均有非常显著差异 (P <0 0 1) ;两组MDA无统计学意义 (P>0 0 5 )。吸入NO和静滴L -Arg比较 ,各指标均无统计学差异 (P >0 0 5 )。结论 :静滴L -Arg治疗 ,可提高患者SOD活性 ,虽然其提高SOD活性的效果并不优于吸入NO ,但经济、简便 ,是一种有前途的治疗HAPE自由荃的代谢的药物
Objective: To investigate the effect of L-arginine on free radical metabolism in high altitude pulmonary edema (HAPE). Methods 8 cases of HAPE (L-Arg group) were treated with intravenous infusion of L-Arg at an altitude of 370 meters and compared with 8 cases of HAPE (NO group) treated with inhaled low concentration of nitric oxide (NO). Before and after treatment Blood levels of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and malondialdehyde (MDA) were measured respectively. Results: The levels of SOD, GSH-Px and MDA in the L-Arg group and the NO group after treatment were significantly higher than those before the treatment (P <0.01). There was no significant difference between the two groups (P> 0.05) . There was no significant difference between inhaled NO and intravenous L-Arg (P> 0.05). Conclusion: Intravenous L-Arg treatment can improve the activity of SOD in patients. Although its effect of increasing SOD activity is not superior to that of inhaled NO, it is economical and simple and is a promising drug for the metabolism of HAPE
