目的:探讨根管直径和锥度对根管充填效果的影响。方法:选取单根管下颌第2前磨牙为研究对象,通过显微CT扫描测量根尖段直径和锥度。根据根尖段直径和锥度分组,选择其中的45颗牙,具体要求如下:与解剖性根尖孔距离为1 mm的根管直径必须<0.3 mm、距离为5 mm的根尖段,其根管锥度应该<0.09 mm,随机分成A、B、C 3组,每组15颗牙。另外再选择其中45颗牙,具体要求为:与解剖性根尖孔距离为1 mm的根管直径应该>0.3 mm、距离为5 mm的根尖段,其根管锥度应该>0.09 mm,以随机方式划分为D、E、F 3个小组,每组各15颗牙。与此同时,再选择10颗牙将其划分为阳性对照组5颗和阴性对照组5颗。实验牙均用Pro-taper镍钛根管锉统一预备至F3号,A、D组实验牙用冷牙胶侧方加压法充填,B、E组实验牙用单尖法充填,C、F组实验牙用热牙胶连续波垂直加压充填,阴、阳性对照组牙不行根管充填,经1%美蓝染色后剖开,观测染料渗入长度。结果:各组微渗漏深度值比较,差异具有统计学意义(P<0.05),两两比较,A组与B组、D组与E组的染料渗入长度差异无统计学意义(P>0.05),A组与C组、B组与C组、F组与C组、A组与D组、B组与E组、D组与F组的染料渗入长度差异有统计学意义(P<0.05)。结论:直径和锥度较大的根管微渗漏较大。热牙胶连续波垂直加压充填根管微渗漏小于冷牙胶侧方加压法和单尖法,冷牙胶侧方加压法和单尖法之间无差别,但单尖法操作更省时。

Results: There was no significant difference in the microleakage depth between groups (P <0.05). There was no significant difference in dye penetration length between group A and B, group D and group E (P> 0.05) ), There was a significant difference in dye infiltration length between group A and group C, group B and group C, group F and group C, group A and group D, group B and group E, group D and group F (P <0.05 ). Conclusions: The microleakage of root canal with larger diameter and taper is larger. Hot denture continuous wave vertical pressure filling root canal microleakage is less than the cold tooth glue lateral pressure method and single-tip method, cold tooth glue lateral pressure method and single-tip method no difference between, but the single-tip method operation More time-saving.
