目的探讨尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)和血清半胱氨酸蛋白酶抑制剂C(Cys C)对妊娠期糖尿病(GDM)肾损伤的早期变化及意义。方法收集新乡医学院第一附属医院85例GDM肾损伤血、尿液标本各5 m L,用酶联免疫吸附试验检测尿NGAL水平;同时用肌氨酸氧化酶法测定血清肌酐(Scr)水平;用免疫透射比浊法测定血清Cys C。结果 85例GDM组尿NGAL和血清Cys C测定结果明显升高,与正常孕妇组尿NGAL和血清Cys C比较,差异均有统计学意义(P<0.05);GDM组与正常孕妇组血清Scr比较,Scr差异均无统计学意义(P>0.05)。85例GDM血清Cys C高于正常者39例(45.88%),尿NGAL高于正常者53例(62.35%),两者联合检测任何一项高于正常者67例(78.82%),所以两者联合检测可以调高阳性率。结论尿NGAL和血清Cys C可作GDM早期肾损伤的诊断标志物。二者联合检测,可以提高阳性率。
Objective To investigate the early changes and clinical significance of urinary neutrophil gelatinase-associated lipocalin (NGAL) and serum cystatin C (Cys C) in gestational diabetes mellitus (GDM). Methods 85 cases of GDM nephropathy blood and urine samples were collected from the First Affiliated Hospital of Xinxiang Medical College for 5 m L each. Urinary NGAL levels were measured by enzyme-linked immunosorbent assay (ELISA). Serum creatinine (Scr) Serum Cys C was measured by immunoturbidimetry. Results The results of urine NGAL and serum Cys C in 85 GDM patients were significantly higher than those in normal pregnant women (P <0.05). Compared with normal pregnant women, serum NGr and serum Cys C were significantly different , Scr difference was not statistically significant (P> 0.05). There were 39 cases (45.88%) of Cys C in 85 cases with GDM, and 53 cases (62.35%) with NGAL in urine were higher than 67 cases (78.82%). Joint detection can increase the positive rate. Conclusions Urinary NGAL and serum Cys C can be used as diagnostic markers for early renal injury of GDM. Joint detection of the two can increase the positive rate.
