目的回顾分析武汉地区老年人群(≥60岁)溃疡性结肠炎的临床特点。方法选取武汉大学中南医院2007年至2011年住院治疗溃疡性结肠炎的患者共237例,对其临床特点、实验室检查结果及结肠镜下表现进行分析。比较老年人溃疡性结肠炎患者(≥60岁)与非老年溃疡性结肠炎患者(<60岁)间是否存在差异。结果 237例溃疡性结肠炎患者中,≥60岁患者38例(16.03%)。在老年溃疡性结肠炎患者中,腹痛、腹泻及发热等表现与非老年患者均无显著性差异(P>0.05)。老年组血便及消瘦发生率高于非老年组,差异具有统计学意义。病变发生部位上,老年患者局限于脾曲以下者94例(47.24%),与非老年组患者存在差异(P=0.036)。按Truelove和Witts UC分度,老年患者重度9例(23.68%),与非老年组差异存在统计学意义(P=0.007)。老年组使用糖皮质激素者14例(36.84%),与非老年组相比差异具有统计学意义(P=0.116)。结论老年溃疡性结肠炎患者与非老年溃疡性结肠炎患者临床表现、病变范围、病情严重程度及治疗方法等均存在一定差异。
Objective To review the clinical features of elderly patients (≥60 years old) with ulcerative colitis in Wuhan. Methods A total of 237 patients with ulcerative colitis hospitalized in Zhongnan Hospital of Wuhan University from 2007 to 2011 were selected for analysis of clinical features, laboratory findings and colonoscopic findings. To compare the differences between elderly patients with ulcerative colitis (≥60 years) and non-elderly patients with ulcerative colitis (<60 years). Results Of the 237 patients with ulcerative colitis, 38 (16.03%) were over 60 years old. In elderly patients with ulcerative colitis, abdominal pain, diarrhea and fever and other non-elderly patients showed no significant difference (P> 0.05). The incidence of bloody stools and weight loss in the elderly group was higher than that in the non-elderly group, the difference was statistically significant. In the lesion site, 94 (47.24%) were elderly patients confined to the spleen below, and there was a difference (P = 0.036) with the non-elderly patients. According to the scores of Truelove and Witts UC, 9 cases (23.68%) were severe in the elderly and there was significant difference with the non-elderly (P = 0.007). There were 14 cases (36.84%) of glucocorticoids in the elderly group, which had statistical significance compared with the non-elderly group (P = 0.116). Conclusion There are some differences in the clinical manifestations, the extent of the disease, the severity of the disease and the treatment methods between the elderly patients with ulcerative colitis and the non-elderly patients with ulcerative colitis.
