本文介绍了血浆脂质、脂蛋白及载脂蛋白的生理作用及正常代谢途径,描绘了肾病时的高脂质,高脂蛋白血症,其异常的机制为肝脏脂质及脂蛋白合成增多及脂蛋白分解代谢减弱。脂质的肾毒性机制为脂蛋白中和肾小球基底膜阴电荷,刺激系膜增殖,损伤肾内小血管,脂蛋白在肾小管内沉积,抑制免疫反应,补体活性亢进和高凝等。本文最后简介了高脂血症的治疗,并认为β-羟-β-甲戊二酰辅酶 A(HMGcoA)抑制剂为有前途的药物
This article describes the physiological role of plasma lipids, lipoproteins and apolipoproteins and normal metabolic pathways, depicting hyperlipidemia, hyperlipoproteinemia in renal disease, the abnormal mechanism of liver lipid and lipoprotein synthesis increased and Lipoprotein catabolism diminished. The mechanism of nephrotoxicity of lipids is to neutralize the negative charge of glomerular basement membrane, stimulate the proliferation of mesangium, damage the small blood vessels in the kidney, deposit the lipoproteins in the renal tubules, inhibit the immune response, hyperfunction of complement activity and hypercoagulability. This article concludes with an overview of the treatment of hyperlipidemia and suggests that β-hydroxy-β-methylglutaryl coenzyme A (HMGcoA) inhibitors are promising drugs
