[目的]观察无锡市社区人群糖代谢异常情况,探讨可能的影响因素及剂量-反应关系。[方法]采用整群抽样方法对无锡市10059例社区居民进行问卷调查、体格检查以及实验室检测,将研究对象按空腹血糖水平分为血糖正常组、空腹血糖受损组和新发现糖尿病组,运用单因素及多因素有序结果Logistic回归探讨影响糖代谢的因素。[结果]10059名调查对象中568人空腹血糖异常,总检出率为5.65%,对性别、年龄进行校正后,单因素分析显示8项指标差异有统计学意义。多因素逐步分析显示年龄、性别、糖尿病家族史、BMI指数、高血压以及高血脂为糖代谢异常的危险因素,文化程度为保护因素。[结论]糖代谢异常受到遗传和环境因素的双重影响,尽早确定糖尿病患病前的高危状态,改变不良生活习性,对控制糖尿病等疾病的发生具有重要意义。
[Objective] To observe the abnormal glucose metabolism in the community of Wuxi and to explore the possible influencing factors and dose-response relationship. [Methods] A total of 10059 community residents in Wuxi were investigated by questionnaires, physical examinations and laboratory tests using cluster sampling method. The subjects were divided into fasting plasma glucose group, fasting plasma glucose group and newly diagnosed diabetic group according to fasting blood glucose level. Using single factor and multivariate Logistic regression results to explore the factors that affect glucose metabolism. [Results] Among the 5,059 subjects, 568 people had abnormal fasting blood glucose, with a total detection rate of 5.65%. After adjusting for gender and age, univariate analysis showed that there were significant differences among the 8 indicators. Multivariate stepwise analysis showed that age, gender, family history of diabetes, BMI, hypertension and hyperlipidemia were risk factors for abnormal glucose metabolism, and the educational level was protective factor. [Conclusion] The abnormal glucose metabolism is affected both by genetic and environmental factors. It is of great significance to control the occurrence of diseases such as diabetes as soon as possible to determine the high risk status of pre-diabetes and to change the bad habits of life.
