目的:观察舒肝养胃汤联合西药治疗消化性溃疡并失血性休克的临床疗效。方法:回顾本院收治的86例消化性溃疡合并失血性休克患者的临床资料,按治疗方式的不同分为对照组41例和观察组45例。对照组在吸氧、抑酸、输血、抗休克等常规处理的基础上给予奥美拉唑肠溶胶囊治疗,观察组在对照组的用药基础上加用舒肝养胃汤治疗。治疗后,观察2组的临床疗效、休克改善率及不良反应发生情况,比较2组患者的止血时间、住院时间以及中医证候积分。结果:治疗总有效率观察组为95.56%,高于对照组的82.93%,2组比较,差异均有统计学意义(P<0.05)。观察组休克改善率为86.67%,对照组休克改善率为73.17%,2组比较,差异有统计学意义(P<0.05)。治疗期间,观察组不良反应发生率为6.67%,对照组不良反应发生率为24.39%,2组比较,差异有统计学意义(P<0.05)。治疗后,观察组的止血时间和住院时间均短于对照组(P<0.05),中医证候积分低于对照组(P<0.05)。结论:舒肝养胃汤联合西药治疗消化性溃疡并失血性休克临床疗效显著,可有效改善消化性溃疡并失血性休克患者的休克症状,缩短止血时间和住院时间,且用药相对安全。
Objective: To observe the clinical efficacy of Shugan Yangwei Decoction combined with western medicine in the treatment of peptic ulcer and hemorrhagic shock. Methods: The clinical data of 86 patients with peptic ulcer and hemorrhagic shock treated in our hospital were retrospectively divided into control group (n = 41) and observation group (n = 45) according to the different treatment methods. The control group was treated with omeprazole enteric-coated capsules on the basis of conventional treatment of oxygen, acid suppression, blood transfusion and anti-shock. The observation group was treated with Shugan Yangwei Decoction on the basis of the control group. After treatment, the clinical efficacy, shock improvement rate and adverse reactions in the two groups were observed. The bleeding time, hospital stay and TCM syndrome scores were compared between the two groups. Results: The total effective rate in the observation group was 95.56%, which was higher than that in the control group (82.93%). There was significant difference between the two groups (P <0.05). The observation group shock improvement rate was 86.67%, the control group shock improvement rate was 73.17%, two groups, the difference was statistically significant (P <0.05). The incidence of adverse reactions in the observation group was 6.67% during the treatment period, and the incidence of adverse reactions in the control group was 24.39%. There was significant difference between the two groups (P <0.05). After treatment, the bleeding time and hospital stay in the observation group were shorter than those in the control group (P <0.05), and the TCM syndrome scores were lower than those in the control group (P <0.05). Conclusion: Shugan Yangwei Decoction combined with western medicine has significant curative effect on peptic ulcer and hemorrhagic shock. It can effectively improve the symptoms of shock, shorten the time of hemostasis and hospital stay in patients with peptic ulcer and hemorrhagic shock, and the medication is relatively safe.
