目的观察不同类型糖尿病前期患者口服葡萄糖耐量试验(OGTT)期间血浆胰高血糖素样肽l(GLP-1)水平变化及其与血糖及血清胰岛素水平的相关性。方法以糖尿病前期患者[单纯空腹血糖异常组(IFG,n=29),单纯糖耐量异常组(IGT,n=16),空腹及餐后血糖均异常组(CGI,n=13)]和健康对照(NGT,n=22)为研究对象。采用OGTT,分别测定空腹和服糖后血糖(PPG)、血清胰岛素(PINS)和血浆GLP-1(PGLP-1)水平。结果 OGTT期间,IGT组和CGI组糖负荷后120 min血胰岛素(PINS_(120))水平显著高于NGT组和IFG组(P<0.05);矫正性别和年龄后,相关性分析结果显示,服糖后30 min,PGLP-1_(30)水平与PPG_(30)和PINS_(30)显著相关(r=0.27,P=0.017;r=0.326,P=0.004);血浆GLP-1曲线下面积与PPG_(30)相关(r=0.223,P=0.049)。结论糖负荷后早期血浆GLP-l水平与血糖水平密切相关,但不同糖尿病前期人群血浆GLP-l水平差异不显著。
Objective To observe the changes of plasma glucagon-like peptide 1 (GLP-1) level in patients with different types of pre-diabetes during oral glucose tolerance test (OGTT) and its correlation with plasma glucose and serum insulin levels. Methods Patients with pre-diabetes (group with simple fasting glucose (IFG, n = 29), group with impaired glucose tolerance (IGT, n = 16), group with abnormal fasting and postprandial blood glucose (CGI, n = 13) Control (NGT, n = 22) as the study. OGTT was used to determine fasting and postprandial glucose (PPG), serum insulin (PINS) and plasma GLP-1 (PGLP-1) levels respectively. Results During the OGTT period, the level of blood insulin (PINS 120) in IGT group and CGI group was significantly higher than that in NGT group and IFG group at 120 min (P <0.05). Correlation analysis showed that after the correction of gender and age, The level of PGLP-1 (30) was significantly correlated with PPG 30 and PINS 30 (r = 0.27, P = 0.017; r = 0.326, P = 0.004) PPG_ (30) (r = 0.223, P = 0.049). Conclusion The level of plasma GLP-1 in the early post-glycogen load phase is closely related to the blood glucose level, but there is no significant difference in the plasma levels of GLP-1 among different pre-diabetic groups.
