目的 探讨脑缺血早期急性肺损伤的病理改变及血浆和支气管肺泡灌洗液中内皮素 1(ET 1)含量变化。方法 用栓线法阻断大鼠右侧大脑中动脉 ,结扎对侧颈总动脉 2 4h ,观察脑与肺病理改变 ,以及血浆和支气管肺泡灌洗液中ET 1含量。结果 脑缺血组光镜下观察可见右侧脑缺血区皮质神经元缺氧变性 ;肺泡隔增宽及中性粒细胞渗出。电镜下观察脑缺血组肺泡II型上皮细胞线粒体肿胀 ,线粒体嵴断裂、消失 ,板层体数量相对减少 ,结构破坏。脑缺血组血浆ET 1(185 .87± 32 .16pg ml)明显增高 ,支气管肺泡灌洗液中ET 1含量呈增高趋势 (38.11± 10 .0pg ml)。结论 大鼠急性脑缺血早期即可出现明显的肺脏病理改变 ,血浆ET 1含量明显增高
Objective To investigate the pathological changes of acute lung injury in early stage of cerebral ischemia and the content of endothelin 1 (ET 1) in plasma and bronchoalveolar lavage fluid. Methods The right middle cerebral artery was occluded by ligament occlusion and the contralateral common carotid artery was ligated for 24 hours. The pathological changes of the brain and lung were observed, and the content of ET 1 in plasma and bronchoalveolar lavage fluid was also observed. Results In the ischemic group, hypoxia degeneration of cortical neurons in the right cerebral ischemic area was observed by light microscopy. The alveolar septa broadened and the exudation of neutrophils was observed. Under electron microscope, the mitochondria swelling and mitochondrial cristae were broken and disappeared in the alveolar type II cells in the ischemic group, and the number of lamellae decreased and the structure was destroyed. ET 1 in cerebral ischemic group increased significantly (185.87 ± 32 .16 pg ml), ET 1 increased in bronchoalveolar lavage fluid (38.11 ± 10 .0 pg ml). Conclusion In the early stage of acute cerebral ischemia, the pathological changes of the lungs are obvious, and the content of plasma ET 1 is significantly increased
