目的:探讨分析动态脑电图监测在病毒性脑炎的诊断及预后评估中的临床价值。方法:选取2010年1月~2012年2月间我院收治的病毒性脑炎患者78例作为研究对象,先后对其进行常规脑电图监测和动态脑电图监测,将两种监测方式检出的轻、中、重度脑电图异常患者例数、检出异常率及预后评估情况进行整理比较,并将患者的临床监测资料进行回顾性的分析。结果:经常规脑电图监测检出的脑电图异常患者有48例,检出异常率为61.5%,经动态脑电图监测检出脑电图异常患者有73例,检出异常率为93.6%,动态脑电图监测的检出异常率明显高于常规脑电图监测的检出异常率,二者差异显著(p<0.05),具有统计学意义。常规脑电图监测与动态脑电图监测检出的重度脑电图异常患者例数、致死及致残率均无显著差异(p>0.05),无统计学意义,检出的中度脑电图异常患者例数及致残率无显著差异(p>0.05),无统计学意义。但是,常规脑电图监测检出脑电图异常的患者,其致死率明显高于动态脑电图监测检出脑电图异常的患者,二者差异显著(p<0.05),具有统计学意义。常规脑电图监测检出脑电图正常的患者,其致残率明显高于动态脑电图监测检出脑电图正常的患者,二者差异显著(p<0.05),具有统计学意义。结论:动态脑电图监测在病毒性脑炎的诊断及预后评估中具有极高的临床价值,值得在临床上推广应用。
Objective: To investigate the clinical value of dynamic EEG monitoring in the diagnosis and prognosis of viral encephalitis. Methods: 78 patients with viral encephalitis who were admitted to our hospital from January 2010 to February 2012 were selected as the research object, and routine EEG monitoring and dynamic EEG monitoring were performed. Two monitoring methods Out of light, moderate and severe cases of abnormal EEG patients, the detection rate of abnormalities and prognosis of the situation were compared, and clinical data were retrospectively analyzed. Results: There were 48 cases of EEG abnormalities detected by routine EEG monitoring, the detection rate was 61.5%, 73 cases of EEG abnormalities detected by dynamic EEG monitoring, the detection rate was 93.6%. The abnormal rate of detection by dynamic electroencephalography was significantly higher than that of conventional electroencephalography (P <0.05), which was statistically significant. There were no significant differences in the number of cases of severe EEG abnormality, death and disability (P> 0.05) between routine EEG monitoring and dynamic EEG monitoring. There was no significant difference between the two groups in the detection of moderate EEG There was no significant difference in the number of cases of abnormal patients and the disability rate (p> 0.05). There was no statistical significance. However, the mortality of patients with abnormal EEG detected by conventional EEG monitoring was significantly higher than that of EEG abnormality detected by EEG monitoring (p <0.05), which was statistically significant . Conventional EEG monitoring detected normal EEG patients, the morbidity was significantly higher than the EEG monitoring detected EEG normal patients, the difference was significant (p <0.05), with statistical significance. Conclusion: EEG monitoring has high clinical value in the diagnosis and prognosis of viral encephalitis, which is worth popularizing in clinic.
