目的对昆明地区1 185例正常儿童运用超声心动图检查测量冠状动脉的内径,并对结果进行初步的分析,探讨其影响因素.方法选择2011年1月至2012年12月在昆明市儿童医就诊的健康儿童1 185例,其中男729例,女456例.除单独分列新生儿组及>12岁组外,其余按照年龄每1岁分为1组,共分14组,排外影响冠状动脉内径变化的疾病,检查前测量各组儿童身高、体重、计算体表面积(BSA).应用超声分别测量主动脉根部内径(AO)、左冠状动脉总干(LCA)、右冠状动脉总干(RCA),计算LCA/AOA(L/A)和RCA/AO(R/A)值.结果采用SPSS统计软件进行统计学分析,男女LCA与RCA均值比较没有统计学意义(P>0.05),LCA与RCA均值间差异比较具有统计学意义(P<0.01),各组间LCA与RCA均值差异比较,F=17.609,P<0.01.通过回归分析,LCA、RCA与体重、身高、年龄、体表面积呈正相关,通过对回归方程进行标准化,结果显示体表面积、身高、年龄与左右冠状动脉内径相关性最好,而体重相比较而言相关性较差.结论冠状动脉内径按照相关性排序,与身高、年龄、体表面积有关,LCA/AO、RCA/AO变化在很小范围内,可以根据其比值作为判断冠状动脉扩张的标准.
