目的观察前列腺素E1不同剂量(40μg和10μg)持续治疗外周动脉病变糖尿病足患者创面愈合的临床效果。方法选取医院收治的外周动脉病变糖尿病足患者72例为研究对象,随机分为大剂量组和常规组,每组36例。大剂量组采用前列素E140μg冲击治疗,常规组采用前列素E110μg常规治疗,治疗后随访1月,比较2组患者创面愈合效果及治疗前后疼痛、踝肱指数。结果大剂量组创面愈合总有效率为97.2%,高于常规组的77.8%(P<0.05)。治疗前2组患者VAS评分及踝肱指数比较无显著差异(P>0.05);治疗后,大剂量组VAS评分低于常规组,踝肱指数高于常规组(P<0.05)。结论大剂量前列素E1在外周动脉病变糖尿病足患者中冲击治疗,可减轻患者疼痛,改善下肢血供,促进创面愈合,具有显著临床疗效。
Objective To observe the clinical effects of different doses of prostaglandin E1 (40μg and 10μg) on the wound healing in patients with peripheral arterial lesions of diabetic foot. Methods Seventy-two patients with peripheral arterial disease and diabetic foot admitted to hospital were selected as study subjects and randomly divided into high-dose group and conventional group, with 36 cases in each group. High-dose group with prostaglandin E140μg impact treatment, conventional group with prostaglandin E110μg conventional treatment, followed up for 1 month after treatment, compared the wound healing effect of two groups of patients and before and after treatment of pain, ankle brachial index. Results The total effective rate of wound healing in high-dose group was 97.2%, which was higher than that in the conventional group (77.8%, P <0.05). There was no significant difference in VAS score and ankle brachial index between the two groups before treatment (P> 0.05). After treatment, the VAS score of high-dose group was lower than that of conventional group, and the ankle-brachial index was higher than that of conventional group (P <0.05). Conclusion High dose of prostaglandin E1 in peripheral arterial disease patients with diabetic foot shock treatment can reduce pain, improve blood supply to lower extremity and promote wound healing, with significant clinical efficacy.
