目的应用相位对比磁共振成像(PCMRI)测量子痫前期(PE)基底动脉(BA)血流动力学参数,探讨PE后部可逆性脑病综合征(PRES)与脑MRI正常患者间BA的血流动力学差异。方法对34例PE患者进行常规头颅MRI扫描,根据常规MRI结果分为PRES组和脑MRI正常组,对两组行BA PCMRI扫描,在工作站上利用流体定量(QFLOW)软件进行后处理,T_1-FFE幅度图像上沿BA边缘绘制感兴趣区,软件自动分析得到心动周期内BA的相关血流动力学参数值及时相-流速曲线,记录BA的血管断面面积、平均流速(MV)、平均流量(MF)及血流阻力(RI)。采用SPSS 17.0统计软件比较两组BA血流动力学参数间的差异。结果 PRES组BA的RI低于脑MRI正常组(t=5.61,P<0.05),血管断面面积、MV、MF均显著大于脑MRI正常组(t_(血管断面面积)=4.54,t_(MV)=6.53,t_(MF)=8.41;P值均<0.01)。结论 PE PRES BA血流灌注呈“低阻高排”特征,BA血流高灌注与PRES的发生密切相关。
Objective To measure the hemodynamic parameters of basilar artery (BA) in preeclampsia (PE) by phase contrast magnetic resonance imaging (PCMRI) and to explore the relationship between BA posterior reversible encephalopathy syndrome (PRES) and normal brain MRI Kinetic differences. Methods 34 cases of PE patients underwent conventional cranial MRI scans. According to the results of conventional MRI, they were divided into PRES group and normal brain MRI group. BA PCMRI scanning was performed on both groups. After treatment with QFLOW software on workstation, T_1 - The region of interest along the BA edge was drawn on the FFE amplitude image. The software automatically analyzed the relevant hemodynamic parameters and the phase-flow curve of BA during the cardiac cycle, and recorded the cross-sectional area, mean velocity (MV) and average flow rate MF) and blood flow resistance (RI). The differences of BA hemodynamic parameters between the two groups were compared using SPSS 17.0 statistical software. Results The RI of PRES in BA group was lower than that of normal brain MRI group (t = 5.61, P <0.05). The cross section area, MV and MF of PRES group were significantly higher than those of normal MRI group (t_ = 6.53, t MF = 8.41, P <0.01). Conclusions The blood flow perfusion of PE PRES BA is characterized by low resistivity and high discharge. The hyperperfusion of BA blood flow is closely related to the occurrence of PRES.
