目的分析2012—2014年北京市西城区托幼机构手足口病密切接触者感染状况及其危险因素,为制定更有针对性的预防、控制措施提供依据。方法收集2012—2014年北京市西城区托幼机构手足口病例密切接触者流行病学信息及病原学检测结果,结合托幼机构消毒隔离效果监测进行分析。结果 485例密切接触者咽拭子标本中手足口病毒核酸检测总阳性率为15.05%,其中以6月份阳性率最高(31.11%);隐性感染与显性感染比例为1.4∶1;阳性标本中Cox A16、EV71、肠道病毒未分型构成比分别为24.66%,20.55%,54.79%。儿童和老师手卫生不合格(OR=5.438)、物体表面消毒效果不合格(OR=4.253)均为肠道病毒感染的危险因素(P值均<0.05)。结论托幼机构手足口病密切接触者肠道病毒感染率较高。需加强手足口密切接触者的管理和宣传教育,普及正确洗手方法,切实落实消毒隔离措施,以减少续发病例和手足口病的暴发和流行。
Objective To analyze the infection status and risk factors of hand, foot and mouth disease in close contact with nurseries in Xicheng District, Beijing from 2012 to 2014 and provide the basis for more targeted prevention and control measures. Methods Epidemiological information and pathogen test results of close contacts of HFMD cases in kindergartens and nurseries of Xicheng District in Beijing during 2012-2014 were collected and analyzed with the monitoring of disinfection and isolation of nurseries. Results The total positive rate of hand-foot-mouth virus nucleic acid in 485 cases of throat swab specimens was 15.05%, of which the positive rate was the highest in June (31.11%). The ratio of latent infection to dominant infection was 1.4:1. Positive specimens In the Cox A16, EV71, the non-typing enterovirus composition ratio was 24.66%, 20.55%, 54.79%. The unqualified hand hygiene of children and teachers (OR = 5.438) and the unqualified disinfection effect on the surface of the children (OR = 4.253) were all risk factors of enterovirus infection (P <0.05). Conclusion Intestinal virus infection rate is high in close contact with hand-foot-mouth disease in nurseries. It is necessary to strengthen the management and publicity and education of close contacts of hand, foot and mouth, popularize the correct method of handwashing, and earnestly implement the disinfection and isolation measures so as to reduce the outbreak and epidemic of recurrence cases and hand-foot-mouth disease.
