目的分析外周血内不同微小RNA(miR)的表达水平对非小细胞肺癌(NSCLC)患者临床预后的影响,为NSCLC患者早期预后判断提供参考依据。方法以同济大学附属上海市肺科医院胸外科收治的184例NSCLC患者为研究对象,按照其术后复发情况分为复发组(n=121)及非复发组(n=63),使用实时荧光定量PCR(q RT-PCR)法检测其外周血miR-19b、miR-21、miR-146a、miR-183、miR-221表达水平,记录两组患者总生存期(OS)并运用单因素及多因素Cox回归分析,计算外周血内不同miR表达水平对患者OS的影响。结果复发组miR-19b、miR-21、miR-146a、miR-221表达水平及高表达率高于未复发组,其miR-183表达水平及高表达率低于未复发组,差异有统计学意义(P<0.05)。Cox回归分析显示,临床分期Ⅲ~Ⅳ期,miR-19b、miR-21、miR-146a、miR-221高表达,miR-183低表达是影响NSCLC患者OS的独立危险因素(P<0.05)。结论外周血内不同miR的表达水平对NSCLC患者复发及OS存在一定影响,有望成为NSCLC患者预后判断的分子标志物。
Objective To analyze the effect of expression of different microRNAs (miRs) in peripheral blood on clinical prognosis of patients with non-small cell lung cancer (NSCLC) and provide a reference for early prognosis judgment in NSCLC patients. Methods A total of 184 NSCLC patients admitted to the Department of Thoracic Surgery, Shanghai Pulmonary Hospital of Tongji University were enrolled in this study. The patients were divided into recurrence group (n = 121) and non-recurrence group (n = 63) The expression levels of miR-19b, miR-21, miR-146a, miR-183 and miR-221 in the peripheral blood were detected by qRT-PCR. The total survival (OS) Multivariate Cox regression analysis was performed to calculate the effect of different miR expression levels on OS in peripheral blood. Results The expression of miR-19b, miR-21, miR-146a, miR-221 and the high expression rate of recurrence group were higher than those of non-recurrence group, the miR-183 expression level and the high expression rate were lower than those without recurrence Significance (P <0.05). Cox regression analysis showed that miR-19b, miR-21, miR-146a and miR-221 were overexpressed and miR-183 overexpression was the independent risk factor for OS in NSCLC patients (P <0.05). Conclusion The expression levels of different miRs in peripheral blood may affect the relapse and OS in NSCLC patients and may be used as a molecular marker for predicting the prognosis of patients with NSCLC.
