目的探讨小猪慢性胰腺炎(CP)MR表现。方法西双版纳小型猪正常7头,CP 15头,其中Ⅰ期7头,Ⅱ期4头,Ⅲ期4头。MR扫描:梯度回波T1W,快速恢复快速自旋回波T2W,MRCP。观察胰腺形态、信号及胰管改变。结果各组间胰腺萎缩无显著差异(p>0.05)。正常组主胰管均未见显示,Ⅰ期CP主胰管平均直径0.6mm,Ⅱ期2.5mm,Ⅲ期为4.5mm。正常组和Ⅰ期CP T1WI胰腺信号等于或高于肝实质,Ⅱ期CP组有3头(75%)信号降低,Ⅲ期CP组胰腺信号均降低(100%)。正常组和Ⅰ期CP组MR各项指标均无显著性差异(p>0.05)。Ⅱ、Ⅲ期CP与正常或Ⅰ期CP T1WI胰腺信号,主胰管直径及分支胰管的显示情况均显著性差异(p<0.05)。主胰管扩张程度与CP严重程度有相关性(r=0.885,p<0.05)。结论常规MR能发现阻塞性CP,并能一定程度予以分期。
Objective To investigate the MR manifestations of chronic pig’s chronic pancreatitis (CP). Methods Xishuangbanna miniature pig had 7 normal and 15 piglets, including 7 in stage Ⅰ, 4 in stage Ⅱ and 4 in stage Ⅲ. MR Scanning: Gradient Echo T1W, Fast Recovery Fast Spin Echo T2W, MRCP. Pancreas morphology, signal and pancreatic duct changes were observed. Results There was no significant difference in pancreatic atrophy between groups (p> 0.05). The main pancreatic duct of the normal group were not shown, the average primary pancreatic duct diameter Ⅰ 0.6mm, Ⅱ 2.5mm, Ⅲ 4.5mm. The signal of pancreas of normal group and CPIWIWI was equal to or higher than that of hepatic parenchyma. The signal of 3 (75%) in group CP was decreased and the signal of pancreas in group III was decreased (100%). There was no significant difference in MR between normal group and CP group (p> 0.05). There was a significant difference (p <0.05) in the signal of pancreatic stage Ⅱ, Ⅲ CP and normal or stage Ⅰ CP T1WI pancreatic signal, the diameter of the main pancreatic duct and the branch pancreatic duct. The degree of dilatation of the main pancreatic duct was correlated with the severity of CP (r = 0.885, p <0.05). Conclusion Conventional MR can detect obstructive CP, and can be staged to some extent.
