目的对报告的职业性正己烷中毒临床病例资料进行汇总分析,总结职业性正己烷中毒发病的一般规律、职业卫生管理及防治方法,为职业性正己烷中毒的预防和控制提供参考。方法通过检索电子文献数据库,收集1994—2013年间报道的1 262例职业性正己烷中毒病例资料,筛选并提取关键数据,采用SPSS 19.0软件进行统计学分析。结果 96.7%中毒病例接触的正己烷浓度超过国家职业接触限值,56.8%中毒病例来自皮革制鞋、玩具制造行业的刷胶工,80.5%劳动者未佩戴任何个人防护用品,84.8%工作场所未配置任何防护设施。64.3%正己烷中毒患者接触时间在3~15个月。神经肌电图在目前中毒的诊断和评估中有重要意义。目前治疗无特效解毒剂,以对症治疗为主,神经营养因子药物治疗效果明显。患者平均病程234.5 d,轻度患者平均治疗时间短于中度和重度患者(U=8 240.5、3 578.0,P<0.05),中度和重度患者治疗时间差异无统计学意义(U=4 588.5,P>0.05)。经治疗后75.6%患者痊愈。结论患者中毒表现为神经系统损害的多发性周围神经病,恢复时间较长,但预后较好。预防和控制职业性正己烷中毒,需要不断完善职业病防治体系,加强预防控制和监督管理。
Objective To summarize and analyze the reported clinical data of occupational n-hexane poisoning, summarize the general rules of occupational n-hexane poisoning, occupational health management and prevention methods, and provide reference for the prevention and control of occupational n-hexane poisoning. Methods A total of 1 262 cases of occupational n-hexane poisoning cases collected from 1994 to 2013 were collected by searching the electronic literature database. The key data were screened and extracted and analyzed by SPSS 19.0 software. Results 96.7% of the poisoned patients had n-hexane concentration exceeding the national occupational exposure limit, 56.8% of them were from rubber workers in leather shoes manufacturing and toy manufacturing industry, 80.5% did not wear any personal protective equipment and 84.8% Configure any protective equipment. 64.3% n-hexane poisoning patients contact time in 3 to 15 months. Neuromuscular mapping is of great importance in the current diagnosis and assessment of poisoning. Currently no effective antidote treatment to symptomatic treatment based, neurotrophic factor drug treatment effect is obvious. The mean duration of treatment was 234.5 days. The average duration of treatment was shorter in mild patients than in moderate and severe patients (U = 8 240.5, 358.0, P <0.05). There was no significant difference in the treatment time between moderate and severe patients (U = 4 588.5 , P> 0.05). After treatment, 75.6% of patients recovered. Conclusions The patient’s poisoning manifests as multiple neuropathies with neurological damage. The recovery time is longer but the prognosis is better. Prevention and control of occupational n-hexane poisoning, we need to constantly improve the occupational disease prevention and control system, strengthen prevention and control and supervision and management.
