目的探讨成人髂腹股沟神经(IIN)的超声表现及临床应用。方法利用超声测量254例志愿者的IIN在穿出腹横肌、腹内斜肌、腹外斜肌腱膜时分别距髂嵴最高点、髂前上棘内侧、耻骨联合上缘的骨性标志点以及至皮肤表皮的距离,并分析其可行性。对比各指标在性别、左右侧、不同BMI组之间的差异。结果 (1)正常IIN在纵切面上呈低回声条状结构,横切面呈圆形低回声。(2)同一或不同医师测量各指标的可行性较好。(3)各指标在不同性别之间差异有统计学意义(P均<0.05);左右侧之间差异无统计学意义(P均>0.05);同一性别不同BMI组之间,3个肌肉穿肌点分别与各骨性标志的距离之间的差异无统计学意义(P均>0.05),而在各肌肉穿肌点与皮肤表皮之间的距离之间差异有统计学意义(P均<0.05)。结论高频超声可清晰显示INN在穿出腹横肌、腹内斜肌、腹外斜肌腱膜时的形态,具有一定的临床价值。
Objective To investigate the ultrasonographic features and clinical application of adult iliovaginal nerve (IIN). Methods The IIN of 254 volunteers was measured from the highest point of iliac crest, the medial part of anterior superior iliac spine and the upper edge of pubic symphysis when penetrating abdominal transversus muscle, abdominal oblique muscle and abdominal oblique aponeurosis. As well as the distance to the skin epidermis and to analyze its feasibility. The differences of each index in gender, left and right, different BMI groups were compared. Results (1) The normal IIN showed a hypoechoic stripe on the longitudinal section and a circular hypoechoic cross section. (2) The same or different physicians to measure the feasibility of each indicator is better. (3) There was a significant difference in each index between different sexes (P <0.05); there was no significant difference between left and right sides (P> 0.05); between the same sex and different BMI groups, 3 muscular wear There were no significant differences in the distances between the muscle points and the bony markers (P> 0.05), while there was a significant difference in the distance between the muscle penetration points and the epidermis (P < 0.05). Conclusion High-frequency ultrasonography can clearly show the morphology of INN when it pierces the transverse abdominal muscle, abdominal oblique muscle and external oblique muscle aponeurosis. It has certain clinical value.
