目的:研究hERG1钾通道在食管鳞癌中的表达及其意义。方法:对68例食管鳞癌患者的癌组织和非癌组织进行免疫组化检测hERG1蛋白表达,并随访;对11例术后食管鳞癌组织以及相对应的非癌食管组织作逆转录PCR检测hERG1 mRNA表达。结果:hERG1蛋白和mRNA在所有食管鳞癌患者的相对应的非癌食管组织中呈阴性表达,而在癌组织中呈异常阳性表达;hERG1蛋白在癌组织中的阳性率为77.9%(53/68),hERG1 mR-NA的阳性率为81.8%(9/11);hERG1蛋白在癌组织中表达水平与临床病理特点比较,各组间的差异均无统计学意义。hERG1阳性表达组平均生存时间明显低于阴性表达组(分别为33.5月和52.8月,P=0.015)。阳性组1,2,3,4,5年生存率分别为88.7%,56.6%,34.0%,15.0%和12.0%,而阴性组为93.3%,73.3%,60.0%,60.0%和44.4%;两组各时间点之间的差异有统计学意义(P=0.033)。Cox模型多因素分析显示,hERG1蛋白的异常表达是影响食管鳞癌患者总体生存率的独立预后因素(P=0.025)。结论:hERG1钾通道在食管鳞癌中异常表达,是影响患者生存率的预后因素。
Objective: To study the expression of hERG1 potassium channel in esophageal squamous cell carcinoma and its significance. Methods: The expression of hERG1 protein in 68 cases of esophageal squamous cell carcinoma was detected by immunohistochemistry. The expression of hERG1 protein in 11 cases of esophageal squamous cell carcinoma and corresponding non-cancerous esophageal tissues were detected by reverse transcription polymerase chain reaction hERG1 mRNA expression. Results: The hERG1 protein and mRNA were negatively expressed in corresponding non-cancerous esophageal tissues of all patients with esophageal squamous cell carcinoma and were negatively expressed in cancerous tissues. The positive rate of hERG1 protein in cancerous tissues was 77.9% (53 / 68). The positive rate of hERG1 mR-NA was 81.8% (9/11). The expression of hERG1 protein in cancer tissues was not significantly different from the clinicopathological features. The average survival time of hERG1 positive expression group was significantly lower than that of negative expression group (P <0.05). The 1, 2, 3, 4, 5-year survival rates of the positive group were 88.7%, 56.6%, 34.0%, 15.0% and 12.0% respectively, while those in the negative group were 93.3%, 73.3%, 60.0%, 60.0% and 44.4%, respectively. The difference between the two time points was statistically significant (P = 0.033). Cox model multivariate analysis showed that abnormal expression of hERG1 protein was an independent prognostic factor affecting the overall survival of patients with esophageal squamous cell carcinoma (P = 0.025). Conclusion: The abnormal expression of hERG1 potassium channel in esophageal squamous cell carcinoma is a prognostic factor affecting the survival rate of patients.
