目的探讨在地中海贫血诊断中应用红细胞平均体积(MCV)、红细胞脆性以及血红蛋白(Hb)电泳联合检测的临床价值。方法选取2013年1月~2014年12月在我院检查的102例地中海贫血患者为病例组,另选同期的102例非地中海贫血患者为对照组,对两组患者均行MCV、红细胞脆性、Hb电泳联合检测,并对检测结果进行统计和分析。结果病例组患者的三种检测方法的单项检测阳性率均明显高于对照组(P<0.05);MCV+红细胞联合检测与MCV单项检测的灵敏度差异较小(P>0.05),其他平行联合检测与各单项检测的灵敏度差异较大(P<0.05),系列联合检测与各单项检测的特异度差异较大(P<0.05)。结论在地中海贫血诊断中应用MCV、红细胞脆性、Hb电泳联合检测,可大大提升诊断特异度和灵敏度。
Objective To investigate the clinical value of combined detection of mean corpuscular volume (MCV), erythrocyte fragility and hemoglobin (Hb) in the diagnosis of thalassemia. Methods A total of 102 cases of thalassemia in our hospital from January 2013 to December 2014 were enrolled in this study. Another 102 non-thalassemia patients were selected as the control group during the same period. MCV, erythrocyte fragility, Hb electrophoresis combined detection, and test results were statistically analyzed. Results The positive rates of single detection of three detection methods were significantly higher in case group than those in control group (P <0.05). There was no significant difference between MCV + RBC and MCV single detection (P> 0.05) The sensitivities of the individual tests were significantly different (P <0.05), and the specificity of the series combined tests was significantly different from that of the individual tests (P <0.05). Conclusion The diagnosis of thalassemia using MCV, red blood cell fragility, Hb electrophoresis combined detection, can greatly enhance the diagnostic specificity and sensitivity.
