异丙基肾上腺素(ISO)是一种合成的儿茶酚胺,可用于心脏停搏、心源性休克、完全性心脏传导阻滞、颈动脉窦症群、慢的心律紊乱和支气管哮喘。ISO用作支气管扩张剂已确定,但在心脏病学中的地位尚未肯定。ISO的临床药理反应完全可预知,但问题在于什么时候需用它则往往不清楚。肾上腺素能β-受体可被ISO所激动,而被心得安所对抗。ISO具有以下作用:增加窦性心率、心肌收缩力、房室传导速度和横纹肌血流;舒张支气管平滑肌、脾包膜、胃肠和子宫肌。口服ISO的疗效不满意。静脉滴注ISO可快速控制心血管效应;舌下含片用于慢性的周身作用;吸入溶液用于支气管局部作用。
Isopropyl-epinephrine (ISO) is a synthetic catecholamine that can be used in cardiac arrest, cardiogenic shock, complete heart block, carotid sinus syndrome, slow heart rhythm disorders and bronchial asthma. The use of ISO as a bronchodilator has been established, but its status in cardiology has not yet been confirmed. The clinical response to ISO is completely predictable, but the question is when it is not always clear when to use it. Adrenergic beta-receptors are stimulated by the ISO, and was as defensive. ISO has the following effects: increase sinus heart rate, myocardial contractility, atrioventricular conduction velocity and striated muscle blood flow; diastole bronchial smooth muscle, spleen capsule, gastrointestinal and uterine muscle. The efficacy of oral ISO is not satisfied. Intravenous infusion of ISO can quickly control cardiovascular effects; sublingual tablets for chronic systemic effects; inhalation solution for bronchial local effects.
